ACTIVITY COMPLETION REPORT


I. IDENTIFYING INFORMATION
Title : 2ND QUARTER PORTFOLIO DAY FOR SCHOOL YEAR 2021-2022
Theme : “Your work, your story!”
Objective :
 To conduct limited face-to-face parent-adviser
conference for feedbacking of student’s performance.
 To distribute and sign student’s report card.
 To display and view student’s output as evidence of their
performance.
Activities :
1. Sending Letter of Request to Barangay Captain for the
limited face-to-face Portfolio Day.
2. Dissemination of information through Messenger and School
Facebook Page.
3. Scheduled Limited face-to-face Portfolio Day

III. SUMMARY/NARRATIVE
Portfolio Day is a culminating activity done after every quarterly examination
wherein the performances of the students during the quarter are being showcased to
parents to dignify the academic performance. This is also the time that the teachers will
talk with each parent about the performance of their child in school.
Pursuant to Division Memorandum No. 038, s. 2022, entitled “New Schedule of
Portfolio Day for the Remaining Quarters of School Year 2021-2022”, where there will be
a simultaneous conduct of 2nd quarter Portfolio Day for Secondary schools in the
Division of Leyte on February 11, 2022.
The information was disseminated through the school’s Facebook page shared
across the platform by the teachers and tagged parents and students. In this time of the
pandemic, social media is the only means of giving out information efficiently and on
time.

The coordinator of the 2nd Quarter Portfolio Day asked permission to conduct
limited face to face meeting from the Barangay chairman through a letter request. The
ever-supportive barangay official signed the petition considering that the school will
strictly implement the health protocols and standards issued by the Inter-Agency Task
Force (IATF) of the Municipality.
To strictly implement the health protocols, parents are scheduled based on their
child’s grade level. Grades 7 and 8 are scheduled at 8:00 to 9:00 in the morning, followed
by the grade 9 and 10 at 9;00 to 10:00 while senior high schools are set at 10:00-11:00
am.
During the conduct of the activity, class advisers and subject teachers were able to
give parents with feedbacks on the performance of their children under this new normal.
Parents in return give their opinions on the status of their children’s schooling at home.
Parents and teachers exchanged thoughts on the means of improving the strategies
regarding this distance modular learning.
After a short kumustahan, report cards are distributed to parents for signing.
Through this, they’ll have a clearer picture on the status of their student’s school
performance. Teachers explained the grading system to parents so that they’ll be able to
catch up the reasons behind every child’s grade. Teachers also pointed out the
importance of taking modules seriously despite this new mode of learning. Parents
played a vital role on executing strategies into action.
Before the parents went home, they’re able to view the outputs of their children
including the learning activity sheets and quizzes. Outstanding students are given a
certificate of recognition as a token for their outstanding performance for the quarter. On
their behalf, their parents received the award.
Despite the disturbance of the weather, the conduct of 2 nd quarter portfolio day is
quite successful. It is because of the continuous support and dedication of the school’s
internal and external stakeholders.
It was adjourned at exactly 11:00 am.
III. ACTUAL RESULT
For this activity, there were 158 parents who rendered their time out of 287
parents for the 2ND QUARTER PORTFOLIO DAY 2022. The following is the breakdown of
attendance per grade level and its corresponding percentage.

IV. MAJOR PROBLEM ENCOUNTERED


Other parents did not attend the Portfolio Day Celebration in our school because
some of them are busy attending on their daily routine at home. There are parents who
excused on attending due to busy and conflict schedules.

V. SOLUTION
Parents who are not able to attend due to personal reasons are scheduled to do
feedbacking on the distribution and retrieval of module. For parents who have
messenger, teachers and advisers opted to call them online to give updates on the
performance of their students.

VI. INSIGHTS/RECOMMENDATION
Parents’ role to the development of a child is to offer a connection to the school to
assist the needs of their child. Parents are partners of teachers in education. They are the
home facilitators and channel. They now have an important task of ensuring their child
receive quality education without compromising their safety.
Parents therefore seek time on attending portfolio day not just on matters concerning
their support on the programs and activities of school but more on the aspect of their child’s
improvement reporting. “ATTENDANCE IS A MUST” policy should be strictly implemented.
And if in the process, a parent needs to attend to an even important need, they must submit
a letter of excuse signifying their reason.
