DEED OF DONATION

  
KNOW ALL MEN BY THESE PRESENTS:

The MUNICIPAL GOVERNMENT OF GLORIA, Oriental Mindoro, a local government unit

created and organized in accordance with the laws of the Philippines, represented by Hon. LORETO S.
PEREZ, Municipal Mayor of Gloria, Oriental Mindoro with residence and postal address at Maligaya,
Gloria, Oriental Mindoro pursuant to a Resolution No. 79 Series of 2015 executed by the
Sangguniang Bayan Members at the Sangguniang Bayan Session Hall – Municipality of Gloria,
Oriental Mindoro dated June 1, 2015 @ 9:00 in the morning, hereinafter called the DONOR,

And

The BUREAU OF FIRE PROTECTION - GLORIA represented by SFO1 HERMINIO

MAPACPAC LAZO, Acting Municipal Fire Marshal of the Municipality of Gloria, Oriental Mindoro
with residence and postal address at Marfrancisco, Pinamalayan, Oriental Mindoro, hereinafter called the
DONEE,

Witnesseth:

That the DONOR is the registered owner of a parcel of land located in Poblacion, Gloria,
Oriental Mindoro and containing an area of FIVE HUNDRED TWENTY SIX (526) SQUARE
METERS under Tax Declaration No. 2015-060001-00937 and Property Index No. 029-06-0001-005-
38 and covered by Transfer Certificate of Title with No. 064-2015002005.

That the DONEE is one of the local government units created and organized in accordance with
the laws of the Philippines, represented by its Acting Municipal Fire Marshal of the Municipality of
Gloria, Oriental Mindoro, SFO1 HERMINIO MAPACPAC LAZO.

That FOR AND IN CONSIDERATION of the service that the DONEE has provided for the DONOR
Municipality, and as an act of gratitude and liberality, the DONOR hereby voluntarily and freely
GIVES, TRANSFERS, and CONVEYS by way of donation the abovementioned property to the
DONEE herein represented by SFO1 HERMINIO MAPACPAC LAZO, Acting Municipal Fire
Marshal of the Municipality of Gloria, Oriental Mindoro, their executors and administrators, all of the
rights, title and interest which the DONORS has in the above-described real property together with all the
improvements found therein free from all liens and encumbrances.

That the DONOR affirms that this donation is not made with intent to deceive her creditors,
and that she has reserved for her sufficient funds and property;

That the DONEE hereby accepts and receives this donation and expresses their appreciation
and gratefulness for the generosity and civic mindedness of the DONOR;

That the DONOR hereby acknowledges notice of this acceptance of this donation by the
DONEE.

IN WITNESS WHEREOF, both the DONOR & DONEE have hereunder subscribed their names
