INDUSTRY SUPERVISED TRAINING GUIDELINES

I. To qualify for the Industry Supervised Training Program, one must:

1. Have satisfactorily completed Practicum I, II and III.


2. Have no academic deficiency aside from concurrent subjects enrolled.
3. Be clear of school accountabilities, financial and other.
4. Be physically fit by the accompanied Medical Certificate certified by a
government physician.
a. ECG
b. HEPA Test for Food Technology
c. Others
5. Accomplish the school waiver.
6. Accomplish the school clearance.
7. Have enrolled for the Practicum/OJT program.

II. To qualify for the Company Assignment, one must:

1. Qualify to Industry Training Program and Policies.


2. Qualify to the company requirements.
3. Be officially Accepted and given job assignments.

III. To satisfactory Comply with the Industry Immersion Training Program, one must:

1. Accomplish the 240 hours on or before ________________ cut off time.


2. Submit a certificate of training on or before __________________ cut off
time. (Failure to do so shall mean exclusion from the list of the candidates
for graduation.)
3. Have a performance rating of at least 5.0 (Satisfactory).
4. Submit the training booklet as part of the academic requirement.
5. Obey the Company Policy, rules and regulations.
6. Uphold the image of the College and the Industry Immersion Training
Program.
7. Not abandon or change his/her official assignment unless given official
permission/sanction by proper authorities. Violation of the above-
mentioned compliance requirements may be ground for unaccredited
Practicum/ OJT activity and disciplinary action.

TRAINING AGREEMENT

This AGREEMENT entered into this 21ST day of JUNE between AURORA

STATE COLLEGE OF TECHNOLOGY, with business address at BRGY. ZABALI,

BALER, AURORA hereinafter referred to as the employer and RENEBOY B.

ATENCIO born on DECEMBER 23, 2001 and residing at BRGY. DIMANAYAT, SAN

LUIS, AURORA and hereinafter referred to as the student Trainee being minor is

represented by RENEBOY B. ATENCIO, hereinafter referred to as the guardian.

WITNESSETH: That the student-trainee undergoing Dual Training Program

System agrees to apply himself diligently, safely and faithfully to work in the company

during the period of the training in conformity with the standard approved for the

aforementioned company and makes parts of this AGREEMENT, and under the

following terms and condition:

1. Period of Training 6 weeks

2. Date of Training ________________

3. Approximate date of Completion of Training ________________

4. Any student trainee who fails to perform satisfactorily within any stages of

his training may be given, at the discretion of the company, a period of one

week within which to improve his/her performance. If within this period the

student-trainee concerned failed to show any improvement he/she should be

dropped from the program.

5. The student-trainee shall work the equivalent of eight (8) hours a day on

week day in the course of his/her training. Time spent in related theoretical

instruction shall be considered as hour training.

6. The company’s rule and regulation shall be observed as part of the terms and

condition of this training.

THAT, the student-trainee who is able to complete the required training period or

has attended at least 85% of it shall be issued a Certificate of Completion.

THAT, the student shall be considered apprentice but and as regular employee

and shall have access to technical training and safety and general information concerning

the area of specialization to the said company.

THAT, a copy of this AGREEMENT shall be furnished without cost to the

student-trainee and the Aurora State College of Technology, Baler, Aurora

THAT, this AGREEMENT shall take effect on the date of the start of the industry

immersion training program by student-trainee.

In WITNESS WHEREOF; the parties have hereunto affixed their signature this

21ST day of JUNE at Baler, Aurora.

JOINT AFFIDAVIT OF CLAIM/WAIVER AND PLEDGE

We, spouse CORAZON S. GAP and RENEBOY B. ATENCIO, SINGLE

all of legal age, Filipino and residence of BRGY. DIMANAYAT, SAN LUIS,

AURORA under oath depose and state that:

1. We, CORAZON S. GAPAS are Guidiance of RENEBOY B. ATENCIO,

2. I, RENEBOY B. ATENCIO am a student of Aurora State College of Technology

and taking up Bachelor of Industrial Technology, Major in AUTOMOTIVE;

3. I, RENEBOY B. ATENCIO, am applying as INDUSTRY TRAINEE of the

AURORA STATE COLLEGE OF TECHNOLOGY, BRGY. ZABALI, BALER,

AURORA

As a requirement of the College Ladderized and Dual Training Program for the

completion of the Degree Bachelor in Industrial Technology, Major in

AUTOMOTIVE.

4. We, understand that being accepted as INDUSTRY TRAINEE will not be


considered as employment, considering that there is no employer employee

relationship for the duration of RENEBOY B. ATENCIO training with the

AURORA STATE COLLEGE OF TECHNOLOGY, BRGY. ZABALI, BALER,

AURORA .

We further understand that such training is only an accommodation to complete

the requirement of the course, RENEBOY B. ATENCIO is taking a part of the Dual

System Training (DTS) of the school.

5. We, CORAZON S. GAPAS hereby agree that our son/daughter be an

INDUSTRY TRAINEE of the AURORA STATE COLLEGE OF

TECHNOLOGY, BRGY. ZABALI, BALER, AURORA.

We therefore waive release and discharge the said entity from any liability, claims

or actions that may arise from such training program, except those which cannot

be waived under existing laws, rules and regulations;

6. We execute this affidavit to attest to the truth of the foregoing statements and for

all other legal intents and purpose it well may serve.

 Grade given by the Industry during the immersion will be recognized as the
Official Grade to reflect in the Grading Sheet as show of faith in respect to the
integrity and independence of the company.
