RELEASE, WAIVER AND QUITCLAIM

cessation of my employment with DV CONSULTING INC. (the “Company”). I further manifest and
declare that:

1. I have already been paid all amounts representing my salaries, back wages, premium pay or
holidays, rest days, overtime pay, holiday pay, allowances, separation pay and other benefits
monetary or otherwise due to me from the Company, its stockholders, officers, directors, agents
and/or its employees, arising out or in connection with my employment with the Company, its
stockholders, officers, directors, agents and/or employees.
2. In consideration of the foregoing payments, I hereby release, remise, waive and forever discharge
the Company, its stockholders, officers, directors, agents and/or its employees, from any and all
action, whether civil, criminal, administrative or otherwise, or from any claim of any kind of
character on account of termination, resignation or cessation of my employment, or in connection
with or arising out of my employment with the Company.
3. I likewise hereby remise, release and forever discharge the Company and its officers, stockholders
and agents, from any and all manner of action, cause of action, sum of money, damages, claims
and demands whatsoever in law or in equity which I had or now have against said entities, their
officers, or assigns.
4. Finally, I have read this document entitled Release Waiver and Quitclaim, and fully understood
every word of it and its meaning and have affixed my signature hereunder voluntarily and freely
with the full and complete knowledge of the meaning and intent of this document and of my rights
under existing laws.
