AFFIDAVIT OF UNDERTAKING

I, ____________, Filipino, of legal age, single and a resident of

________________________, Philippines, after having been sworn in
accordance with law, hereby depose and state that:

1. I was recently admitted to the Philippine Bar on _________. A copy of

the Certificate of Membership is hereto attached as Exhibit “A”;

2. On _____________, I applied for the issuance of the certificate of

good standing/certificate of no pending case with the Office of the Bar
Confidant through its online portal;

3. Upon inquiry with the Office of the Bar Confidant, the issuance of the
certificate of good standing/certificate of no pending case will take
more or less two (2) months.

4. Considering the length of time for its issuance and for purposes of
applying for Notarial Commission, I hereby undertake that I will
immediately submit the certificate of good standing/certificate of no
pending case to the Regional Trial Court (OCC) upon receipt from the
Office of the Bar Confidant.

5. I am executing this affidavit to attest to the truth of the foregoing

facts and for whatever legal purposes it might serve.
