MEMORANDUM OF AGREEMENT (MOA)

This Memorandum of Agreement entered into and executed this 10th day of May 2022
at Macabebe Elementary School, Sapang Pari,Sn.Roque, Macabebe, Pampanga

By and Between

MACABEBE ELEMENTARY SCHOOL

MES GRADE III PARENTS represented by NEOME M. RETADA HRPTA PRESIDENT of the
Grade III Class.

WHEREAS the STAKEHOLDERS shall support and initiate by donating materials for structuring
and beautification of the classrrom.

WHEREAS one of stakeholders,ENGR.MICHAEL M. CHAVEZ donate Php 6,000 cash for

the 3 gal of Floor Coating Paint and Labor for the painting.

This agreement takes effect on May 10, 2022.

IN WITNESS WHEREOF, the parties aforementioned have set their hands on the date and
placed abovementioned.

MES GRADE III-RIZAL PARENTS represented by NOEME M. RETADA ,HRPTA President

in Grade III.

WHEREAS one of stakeholders,ENGR.MICHAEL M. CHAVEZ donate Php 6,000 cash for

the 3 gal of Floor Coating Paint and Labor for the painting.

WHEREAS the Grade 3 Adviser with the consent of the parents shall take responsibility on how
to use the materials.

This agreement takes effect on May 10, 2022.

The HOMEROOM PARENTS AND TEACHERS’ ASSOCIATION of MACABEBE ELEMENTARY SCHOOL

an association with the address at Sapang Pari,Sn.Roque, Macabebe, Pampanga, represented
herein by its HRPTA President,NOEME M. RETADA, herein referred to as “STAKEHOLDER/S”.
WITNESSETH: That,
WHEREAS, with the 2021 Brigada Eskwela and Oplan Balik Eskwela, Amending Thereby
DepEd Memorandum No. 032, s. 2020 titled 2020 Brigada Eskwela Program Implementing
Guidelines and DepEd Memorandum No. 014, s. 2020 titled 2020 Oplal Balik Eskwela, the
DONEE acknowledges the need for partners in the opening of classes and Brigada Eskwela 2021
and the whole year round.

WHEREAS the STAKEHOLDERS shall support and initiate by donating printing materials,
health and sanitizing equipment or essentials for COVID19.
This agreement takes effect on May 10, 2022.
