    AFFIDAVIT OF LOSS
    I, ADORA ALIPIO CAYAT, of legal age, widow, Filipino Citizen with residence and
    postal address at #845 Obulan, Beckel, La Trinidad, Philippines, after having been duly
    sworn to in accordance with law do hereby depose and state that:

    1. I am one of the biological daughter of the late SAB-IT ALIPIO who died on May 4,
    2004 at Baguio General Hospital and Medical Center, Baguio City;

    2. When my father died I and my siblings thought that the hospital will be the one
    who will register the facts of his death with the Civil Registrar of Baguio City and
    we did not pay much attention to such matter because we were mourning at that
    time and we do not know what to do;

    3. What I recall was that during that time I and my siblings just pick-up the dead
    body of our late father and we did not sign any document regarding the Death
    Certificate;

    4. If there’s any document that contains the details of death of our late father, if
    there’s any, maybe it might have been lost considering the years that passed by
    and even if we will exert our diligent effort still we could not find it;

    5. I am executing this affidavit to attest to the truth of my foregoing statements and


    to state the fact for all legal intents and purposes it may serve.

    IN WITNESS WHEREOF, I have hereunto set my hand this 25 th day of June, 2021, at
    Baguio City, Philippines.
