JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

WE, JOSE JALIMAO DOMINGO, JR,of legal age, Filipino, and presently
residing at J.P Laurel Avenue, Bajada, Davao City, and ROMAN PACQUING, of legal
age, Filipino, married and presently residing at Shrine Hills, Matina, Davao City, after
being duly sworn to in accordance with law, depose and state that:

1. We personally know CESAREO LIMJAP NAZARIO;

2. As such, we have personal knowledge and were fully aware of the fact that
LESARIO NAZARIO, JOSE NAZARIO and JOSE CESAREO, refers to one and the
same person;

4. That in his Certificate of Live Birth, Marriage Contract and in his SSS record, his
name therein was erroneously written and reported as CESAREO NAZARIO, JOSE
NAZARIO and JOSE CESAREO. Nonetheless, these three names still refers to one
and the same person in the name of CESAREO LIMJAP NAZARIO; AND

4. We are executing this affidavit to confirm and affirm the foregoing facts and for
whatever legal purpose it may serve.
