    STATUTORY DECLARATION

    I, MUSHIKOMA ANDREW WAFULA PAUL of ℅ M/s FIDES LEGAL ADVOCATES


    Plot 1-3 Coral Crescent off Lower Kololo Terrace P.O Box 11529, Kampala-
    Uganda, do solemnly make oath and state as hereunder: -

    1. THAT I am a male adult Ugandan of sound mind and the declarant hereof.

    2. THAT I was informed by my parents which information I verily believe to be true


    that I was born on the 04th day of October 1968, to Mr. Dickson Wamai and Mrs.
    Loyce Masawi, at Bunashimolo Village, Nampanja Parish, Buwalasi Subcounty,
    present day Sironko District of Uganda.

    3. THAT I was also informed by my said parents, and which information I verily
    believe to be true that upon my birth I was given the name MUSHIKOMA
    ANDREW WAFULA PAUL.

    4. THAT I am also a holder of a Ugandan National Identity Card No.


    CM68051102RWXG where my name is entered as MUSHIKOMA ANDREW,
    and my date of birth entered as 04 th October 1968. (A copy of my said Identity
    Card is hereto attached and marked as Annexture “A”).

    5. That I am also a holder of National Social Security Fund Card No.


    6917900002558, where my name is entered thereon as WAFULA PAUL
    MUSHIKOMA, and my date of birth as 05th December 1969. (A copy of my said
    NSSF Card is hereto attached and marked as Annexture “B”).

    6. That the names MUSHIKOMA ANDREW and WAFULA PAUL MUSHIKOMA as


    they appear on my said identification and official documents relate and refer to
    the same person and that is me, and my correct date of birth is the one that
    appears on my national identification card.

    7. THAT I do swear this Statutory Declaration in verification of my name and date of


    birth, conscientiously believing the same to be true by virtue of the provisions of
    the Statutory Declarations Act, Cap 22.

    8. THAT whatever I have stated hereinabove is true and correct to the best of my
    knowledge and belief, EXCEPT for the information in paragraphs 2 and 3 which I
    obtained from my said parents.
