Sto Nino, Guizo, Mandaue City, 6014, Philippines, after having been duly sworn to in accordance with
law, do hereby depose and say:

That I am the manager of SAMPHIL WOOD INDUSTRIES CORPORATION with business address
located at DB Neri St. Sto Nino, Guizo, Mandaue City, 6014, Philippines

That I am the lawful owner of 1 crate said to contain DOUBLE SURFACE PLANER MODEL GT
400BRD set to arrive in Port of Cebu on July 18, 2017 from TAIWAN;

That the said shipment does not contain any contraband articles

That this is my first my first and last shipment that I will file in the Bureau of Customs because I
am not a regular importer.

That I am this affidavit to attest the truth of the foregoing statement and for all legal intent to
the purpose it may serve.
