Questionnaire for Students with Chronic illnesses

Name of school: Zamboanga Del Sur Senior High School

Grade Level: 12

2. How does cancer start? Or how long have you been diagnosed with Chronic Illness?

Demsie: I have diagnosed for 7 years as Acute lymphoblastic leukemia during my 3 rd grade

of elementary in Balongating Elementary School.

3. How can you describe your situation with your academic performances?

Demise: My academic performance was very poor I can’t concentrate my academic in school

because of weaken body, bones pain, chemotherapy and affected my mentality aspect as well

being frustrated all times as I endure my condition.

4. Does your long-term illness condition affect your attendance and participation at school?

Demise: Yes, specially schedule of my chemotherapy, biopsy in Cebu doc every Month of

the year.

5. In your condition does it possible to happen during class hours? Or how do you feel

mostly feel sick?

Demsie: No, actually we agreed from my school that I should take home-based intervention

instead of actual school which is very risky for me to attain.

6. Have you received any advice from your teacher or classmates knowing that you have a
long-term illness?

Demise: Yes, they were very kind towards me they always motivate me whenever I felt

giving up my dreams in life. My teacher also very considerate incomes of passing my task

and giving me allotted time in taking all subjects.

7. Is there any instances that your condition can be hindrance in your academic school

performances?

Demise: Yes, definitely

8. How are you dealing with the challenges during your school performance or your

academic performances?

Demsie: I always be fighter and trusting God, of course with the support of my family,

teachers and classmates help me to overcome the challenges. My parents and my teacher

as well don’t pressure me in handling academic performances if I felt irritated like I don’t

get the instructions I learn to calm my self , try to figure out what is being ask and

limiting my self to what only I can do with regards to my academic for me not to trigger

incase of coping up any challenges.

9. Do you think being a home-based student makes you feel the same when you are in

school base?

Demsie; No, aside from not having a classmates, seatmates and teachers all the time due to

my conditions no interactions and participation during home based- intervention.

10. Calling yourself as a survivor, how do you survive with all your academic school

performances during your long-term illness?


Demise: Perseverance, big faith and support is very important specially from the parent.
