CELLPHONE PAYMENT PLAN AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This Agreement is entered by and between:

_______________, of legal age, Filipino, single and a resident of Block 22, Lot
17 Chrysanthemum St. Pinewood Subdivision, Barangay San Jose, Malaybalay
City, Bukidnon 8700 (hereinafter referred to as “DEBTOR”);

-and-

________________, of legal age, Filipino, single and a resident of Block 7, Lot

12 Greentown Villas 1 Ext. Annex Barangay Mambog III, Bacoor City, Cavite
4102(hereinafter referred to as “CREDITOR”);

WITNESSETH: That-
WHEREAS, the DEBTOR has an existing obligation to the CREDITOR for an amount of P_____ as a payment
to the cellphone loan ____________________;
WHEREAS, for and in consideration of the foregoing premises, the parties hereto agree as follows:
1. The PARTIES hereby agree to the schedule PAYMENT PLAN.

2. The DEBTOR shall conform to the schedule set and shall pay to the CREDITOR before or upon due
the amount as indicated in the PAYMENT PLAN.

3. Payment shall be made to the CREDITOR thru ___________________.

4. Should the DEBTOR delay his/her payment on the due date, the DEBTOR shall be obliged to pay an
additional P 100.00 per day starting from the due date in a billing month. If after 10 days from due date
and the DEBTOR shall still delays his/her payment, the CREDITOR shall withdraw the cellphone unit
received by the DEBTOR in this CELLPHONE PAYMENT PLAN AGREEMENT (_______________).
5. If for any reason the DEBTOR should not oblige to pay the total amount in due date of this Agreement,
the DEBTOR shall be considered in default. Under such event, the remaining balance of the Amount
Owed shall be due within five (5) business days.

IN WITNESS WHEREOF, the Parties has executed this Agreement, on this ____ of _________ at Malaybalay
City, Bukidnon.
