HEALTH PATTERN BEFORE
Health Perception Patient thinks she was healthy before
hospitalization despite being diagnosed
of SLE in 2017. However, she overworks
herself in doing household chores.
Whenever the patient experiences
common cold, fever, and body pain, she
self-medicate with OTC drugs such as;
paracetamol, mefenamic acid, and bioflu.
The patient only seeks medical advice to
a doctor or healthcare professionals
when condition worsens and becomes
unmanageable. Also, the patient has
complete immunizations with two dozes
of SINOVAC COVID- 19 vaccine but no
booster vaccine yet. The patient had no
past surgeries. The patient considers
doing household chores as her exercise
and she does not have any vices.
Elimination The patient voids 3-5 times a day with
light yellow urine. She does not feel any
pain or discomfort during urination. She
also defecates 1-2 times before
hospitalization. She rarely experiences
diarrhea. Patient is not constipated.
Activity - Exercise She considers doing household chores as During her stay in CVMC-Medical Ward,

of SLE, she feels that she is weak and
unhealthy. The patient considers health
as the state of having no illness or
condition that obstructs activities of daily
living and being able to do tasks
efficiently and effectively.
During hospitalization, the client urinates
more often and verbalized “Hindi ko po
nabibilang kung ilang beses po ako
umihi”. The patient was prescribed
Furosemide which is a diuretic drug which
causes her to urinate more often. She still
defecates once or twice during her
hospitalization.

Cognitive-Perceptual
Patient J.B.C is coherent and well-aware
of time, place, situation, and responsesto
stimuli. She speaks Ilocano, Tagalog, and
English. She is also able to read and write
and comprehends very well. Her
education attainment is high school
graduate.
Patient is conscious, oriented, and
communicates very well in Ilocano and
Tagalog. She does not have any problems
with her senses. She is well-oriented of
the procedures done to her and she is
capable of deciding for herself.

Sleep-Rest Patient J.B.C sleeps 6 hours a day on the
average. She sleeps at around 7 in the
evening because of tiredness and wakes
up at about 1:00 in the morning. After
waking up, she still lies in bed until she
gets sleepy again and wakes up at 5 in the
morning to start her usual routines. After
eating dinner, she would bathe herself to
prepare to sleep. She sleeps with her
husband and she does not have difficulty
sleeping before hospitalization. She take
an afternoon nap around 1-2 in the
afternoon after their lunch.
During her stay in the hospital, the
patient does not have any difficulty
sleeping. She stated that she was given
adequate rest periods. The patient stated
that “Hindi naman ako naiistorbo sa
paligid ko.” She also stated that, “Mas
nakakarest ako at nakakatulog dito kasi
wala akong iniisip na trabaho”. It also
does not bother her when nurses wakes
her up for monitoring and giving
medications. Patient J.B.C takes naps
during rest periods.

Self-perception – Self-concept She sees herself as a very hardworking Patient verbalized feeling of sadness
and caring housewife, mother, and because she still cannot go back to her
grandmother. She said that she is well- usual routines. She does not have any

grandchildren. Patient J.B.C is also
contented with her life and very happy
because she has a well-loving family.

Roles-Relationship Patient J.B.C is a housewife, mother, and
a grandmother. She lives in an extended

where she said she is well-loved and
appreciated. She stated that her husband
decides for the family but in every
decisions he makes she is also consulted
before making a decision.
During her stay in CVMC, she said that”
Feeling ko hindi ko nagagampanan ng
maayos yung gawain ko sa bahay”. Her
daughter takes care of her at the bedside.
She stated that she does not have any
problem with her relationship with her
husband and other members of the
family.
