Re: Explanation Letter on Mr. Augusto S. Ortiz complaint dated August 8,

2022

Sir,

This has reference to the Complaint Letter of Mr. Ortiz relative the incident on
August 2, 2022

I was then on duty at the time when Mr. Ortiz was brought to the ER and I
vehemently deny the allegations in his complaint that no one attended to him
and that we laughed at his condition. The factual antecedents of what
transpired that day is reflected in my Incident Report (herein attached);

It was in good faith that the attention of Dra Saludez was called as it was one
of the hospital staff that mentioned Mr. Ortiz as the former’s father. It was out
of compassion and genuine concern that her attention was called for. If she
was offended and left Mr Ortiz instead of attending to him, that was her call.

I was on duty alone at that time and I had done all my chores diligently and to
say that I laughed at Mr. Ortiz’s condition is uncalled for. As the duty nurse, I
have not received any complaint except this exaggeration of events that day
when Mr. Ortiz was brought into the ER. He can attest to the facts that I did
him no harm and he was attended to promptly

My IR will attest likewise that I have not done anything beyond what was called
of my duty as the ER nurse. I am accountable of my own actions and I cannot
speak or can be held accountable to the acts of omission or commission of
others.

In case of an investigation, I am willing to submit myself and will be made

available when necessary.
