 Type the following letter in Times new roman size 12


 Input the text accurately, no spelling mistakes
 Use Open Punctuation
 Apply single line spacing
 Use fully blocked paragraph styles

Type the following Doctor name, Medical Dr Alan Wood MB BCH NUI
name and address in Times New Roman, Royal Victoria Eye and Ear Hospital
Bold. The company name is size 14 and Adelaide Road
the address and phone are size 12. Dublin 2

Centre this at the top of the document and
add a bottom border.

 You work for Dr Alan Wood MB BCH NU, consultant and you have been asked to send a letter to:
Dr Niall Pender MICGP, Clane Medical Centre, Clane, Co. Kildare

 Your company’s reference for the letter is AW/SF and the subject of the letter is
Sarah Wearing, 92 Beech Avenue, Clane, Co. Kildare. DOB: 12-04-11. The letter subject
should be uppercase and bold.

 The letter should be dated today in long date format and will be signed by

Letter Text

It is now a year since Sarah had bilateral grommets inserted into her tympanic membranes,
during which time she has been well. However, about six weeks ago she had an attack of otitis
media. (New Paragraph) She returned to the clinic again yesterday for a review when I found
that the grommets had been dislodged from the tympanic membranes and were lying in the
meatuses from where I removed them. I found that the right middle ear was aerated but on the
left side she had a middle ear effusion. Although this was within normal limits, she had a mild
loss of 20 decibels on the left side. (New Paragraph) I have asked Mrs. Wearing to make sure
that Sarah frequently practices Valsalva’s manoeuvre and I reminded mother and daughter how
important this exercise is in returning Sarah’s ears and hearing to normal function. (New
Paragraph) I will review the situation again in three months.
