NARRATIVE REPORT

“Grade11 H.E. Junior Chef - Election of HPTA Officers SY 2021-2022”

Last October 30, 2021, the election of HPTA was done through online Poll
Questions voting A total number of 33 Parents/Guardians participated on the said
event activity.

The Elected Grade 11 H.E. Junoir Chef HPTA Officers s.y. 2021-2022 are;
President-Mrs. Maria Liza Alferos , Vice President- Mrs. Fredilita A.Donato ,
Secretary- Mrs. Daisy Anova, Treasurer-Mrs. Eufemia Alferos, Auditor-Mrs. Olga
Facelo, Bus.Manager-Mrs. Felixmenia Borja, Sgt. At Arms-Mr. Raulen F. Alferos,
P.I.O. -Mrs. Daisylyn B. Garcia Peace Officer-Mrs. Virlyn M. Decano.

During the next distribution and retrieval of modules I ask the elected HPTA
officers to sign my printed List of HPTA Officers as confirmation and acceptance of
their position that is being elected though online voting.
