ACCOMPLISHMENT COMPLETION REPORT

ON THE SCHOOL HEAD’S REPORT ON THE PROGRESS OF SIP/AIP IMPLEMENTATION
March 23, 2018

I. Title: School Head’s Report on the Progress Of SIP/AIP Implementation

II. Introduction: A leadership network communication between and among school and community leaders for
informed decision-making and solving of school-community wide-learning problems. A network has been established
and is continuously improved by the school community.

III. Objectives: To improve learning outcomes.

IV. Activities:

The activity started at 1 o’clock in the afternoon with an opening program, a prayer led by
Almae Ballacar, Grade 4 teacher, conducting of Philippine National Anthem by Jessica B. Ramelo,
then a Panunumpa sa Katapatan ng Watawat ng Pilipinas lead by Johnsel Frans Escoreal, Grade
5 pupil. Followed by attendance check.
A welcome message was given by the GPTA President, Nimfa Leona.
The school head Jenny A. Maasin, reported to the stakeholders the SIP/AIP
Implementation Plan and Progress on SIP. She emphasize the increase of the enrolment, the zero
failure rate and drop out rate, the decrease of non-readers and non-numerates, the achievements
of the school, the contest win by the pupils, the improvement of the school facilities and the
programs, projects and activities done in school. She emphasize that these wouldn’t happen
without the help of all stakeholders. She also present the school plans for the next school year.
Closing message was given by the Brgy Chairman, Francisco Gerona.

V. Problems Met: Did not attain the 100% attendance.

VI. Resolutions/Recommendations: Give certificates recognition to stakeholders who participated in the activity so
that they will be motivated to attend.
