1. INTRODUCTION

1.1. This tender calls for A Two (2) Years Contract for Non-Destructive Testing (NDT) Service for HRSG &
Boiler’s Pressure and Non-Pressure Parts Repair Works at TNB Repair And Maintenance Sdn. Bhd.
(TNB REMACO) O&M Southern Power Generation (SPG) Pasir Gudang, Johor. The actual usage will
be subjected to assessment during the outage.

2. SCOPE OF WORKS

2.1. The work requirements SHALL be taken as the minimum estimated works scope and SHALL not be
read as limiting the extent of works. It is not the intent to specify completely herein all details of
works required. However, the works SHALL conform in all aspects to high standards of engineering
and workmanship and SHALL be capable of performing in a manner acceptable to TNB REMACO
O&M SPG. The tender is to include the whole works that are described in or implied by the tender
document. All matter omitted from the tender document which may be considered to be necessary
for the efficiency, reliability and completion of the works, SHALL be deemed to be included in the
Tender Price. In view of tight outage schedules, TNB REMACO O&M SPG and the contractor will have
discussion to finalize actual work scopes to be performed, prior to Contractor’s mobilization to site.

2.2. The work SHALL comply with requirements listed in The Factories & Machinery Act, 1967 and other
related statutory requirements.

2.3. FOR CONFINED SPACE WORKS: Any work involving confined space by contractor SHALL follows
Industry Code of Practice for Safe Working in A Confined Space 2010 and any superior code of
practices by DOSH. The manpower must comply with Authorized Entrant Standby Person (AESP)
requirements. It is also contractor’s duty to ensure adequate lighting and ventilation. TNB REMACO
O&M SPG has the right to stop the work if found that the manner of carrying out the work is
dangerous or there is no proper lighting and ventilation installed. If so, contractor must correct or
rectify the problem before resume to work. The delay cost due to safety problem SHALL be borne
by contractors.

2.4. The test method SHALL cover the performance of NDT on any filler or butt weld with the associated
heat affected zone on the parent metal and components. The reference code(s) shall be (but not
limited to):

a) Non-destructive examination in accordance to the requirements of ASME Section V


b) Pressure Vessels in accordance to the requirements of ASME Section VII
c) Power Boilers in accordance to the requirements of ASME Section I
d) Any other relevant codes specified by the Engineer-In-Charge

2.5 NDT PERSONNEL MINIMUM QUALIFICATION/CERTIFICATION


2.5.1 The NDT personnel shall be certified/qualified by ASNT-SNT-TC-1A or PCN (Level I for operators
and Level II for technicians), CSWIP 3.1 – Welding Inspector and API 570- Piping Inspector. Other
equivalent qualification shall only be considered if deemed equivalent.

2.5.2 Station may request NDT Inspector if more than 1 NDT inspection being performed at the same
time. The NDT inspector is responsible to oversee all NDT inspection.

2.5.3 Contractor MUST submit the detail name list with the respective certification level and the CV in
the tender submission.

2.6. Contractors MUST submit the detail of manpower list as below:


a) Dye Penetrant Test (DPT) – 2 teams
b) Ultrasonic Thickness Test – 2 teams
c) Ultrasonic Flaw Detection – 2 teams
d) Phased Array Ultrasonic Test (PAUT) – 2 teams
e) Radiographic Test (RT) – 2 teams
f) Videoscope – 1 team
g) Hardness Test (HT) – 1 team

2.7 QUALITY PROCEDURE/WORK INSTRUCTION

2.7.1 Contractor to provide the Quality Procedure/Work Instruction during tender submission. The
procedure/work instruction should cover the following NDT:
a) Dye Penetrant Test (DPT)
b) Ultrasonic Thickness Test
c) Ultrasonic Flaw Detection
d) Phased Array Ultrasonic Test (PAUT)
e) Radiographic Test (RT)
f) Videoscope
g) Hardness Test (HT)

2.7.2 TNB REMACO O&M SPG in-charge Sr. Engineer or his representative shall verify the
procedure/work instruction. The procedure shall specify the method on (but not limited to):
a) Cleaning process
b) Application process
c) Examination of results
d) Post cleaning whenever needed.
The performance of the NDT shall comply to this endorsed procedure/work instruction.

2.8 EQUIPMENT
2.8.1 Contractor to provide the list of the equipment available during tender submission. The equipment
& typical product should cover the following NDT (if applicable):
a) Ultrasonic Thickness Test
b) Ultrasonic Flaw Detection
c) Phased Array Ultrasonic Test (PAUT)
d) Radiographic Test (RT)
e) Videoscope
f) Hardness Test (HT)

2.8.2. All works SHALL be carried out on a 24-hour basis EXCEPT where specifically stated otherwise.
2.8.3. The Contractor SHALL supply the required manpower, consumables, transportation, tools and
equipment for the purpose of execution of the under mentioned WORKS EXCEPT where specifically
stated otherwise.
2.8.4. The Contractor SHALL follow TNB REMACO O&M SPG in-charge Sr. Engineer or his representative
instruction in ensuring the works are carried out according to the specification.
2.8.5. To arrange for a safety briefing and carry out the Job Safety Assessment/HIRADC. Approval SHALL
be obtained prior to work commencement.
2.8.6. To carry out daily safety tool-box talk before commencing the job and to keep all the attendance
list.
2.8.7. The NDT SHALL be performed by qualified personnel. The contractor shall provide the identity and
qualification of the personnel prior to work commencement.
2.8.8. Contractor to mark completion area with suitable color markings (paints, sprays etc.) as agreed by
TNB REMACO O&M SPG in-charge Sr. Engineer or his representative to ease final inspection.
2.8.9. To do housekeeping after work completion. All disposals, if required, SHALL be arranged by
contractor.
2.8.10. Joint inspection with TNB REMACO O&M SPG in-charge Sr. Engineer or his representative before
the issuance of work clearance.
2.8.11. Contractor SHALL inform and acquire approval from TNB REMACO O&M SPG BMS Sr. Engineers
and Competent Person on the number of manpower and the reason before mobilizing under the
schedules. If TNB REMACO O&M SPG Sr. Engineers are uninformed on the mobilization and details
prior to execution, claims will not be entertained and payments will not be processed.
2.8.12. TNB REMACO O&M SPG holds the right to instruct contractor to utilize one/a team of manpower
for multiple tasks at site without the need for multiple mobilizations whenever necessary.
2.8.13. All the claims under any of the mobilizations (timesheets) SHALL be signed and stamped by TNB
REMACO O&M SPG site supervisors and verify Sr. Engineers by during and after the works. Claims
without signature and stamp will not be entertained.

3 INSPECTION AND TEST PLAN

3.1. For the work scope detailed in paragraph 2.0, the contractor must strictly adhere to the
Inspection and Test Plan (ITP). The objective of the ITP is for TNB REMACO O&M SPG able to
ensure the quality while monitoring the progress of the works. Contractor SHALL provide the
ITP prior to work.
3.2. For Witness and Hold Point activities, contractor SHALL notify TNB REMACO O&M SPG Sr.
Engineer or Competent Person responsible for the works a day before the inspection the latest
being before 1600 hours.
3.3. Both parties, the contractor’s Supervisor and TNB REMACO O&M SPG assigned person for the
works are required to verify the completion and acceptance of the inspection by completing the
Inspection Result Sheet.
3.4. If there is any rejection due to non-conformance of the works based either on work scope, or
acceptance criteria, or applicable specifications, Standards or method statements / work
procedures, TNB REMACO O&M SPG assigned person will note down the reason/s of the
rejection on the aforementioned form. The contractor is then required to rectify the works to

comply with the requirements/specifications and re-inspection SHALL be carried out


accordingly until it is accepted.
3.5. Contractor MUST submit sample ITP during tender submission.

4 WORK PROGRAM AND MOBILIIZATIONS

5.1. Contractor SHALL be prepared to mobilize within 3 days after notification from TNB REMACO
O&M SPG and 24-hours after notification from TNB REMACO O&M SPG for urgent work. The
contractor’s personnel must undergo SPG Safety Briefing prior work commencement and must
adhere to all safety rules and ISO14001 EMS/ ISO55001 AMS requirements regulations at all
time.
5.2. The contractor’s tools and equipment are subjected to Safety Tools Inspection by TNB REMACO
O&M SPG Safety Personnel before permitted to be brought into SPG’s compound. If any tools
found being used without undergoing Safety Tools Inspection will be confiscated. Contractor to
ensure their tools are fit for purpose.
5.3. It is the utmost responsibility of the contractor to ensure the safety of its workers. All workers
must possess valid NIOSH-TNB Safety Passport/OGSP, valid AESP & medical checkup from OHD
(for working inside confine space) and valid insurance during the works.
5.4. It is MANDATORY for the contractor to ensure there is minimum one (1) confined space
STANDBY PERSON in charge at respective work locations that involve confined space and
must possess valid AESP.
5.5. Covid-19 related requirement - the Covid test are subjected to TNB REMACO O&M SPG
requirement at that time.
5.6. List of tools and personnel names SHALL be submitted in advance for security & safety purposes.
5.7. Any delays in completion of the WORKS, action SHALL be taken by the TNB REMACO O&M SPG
according to General Conditions of Tender (Syarat - syarat Am Tender).
5.8. Result for NDT need to notify TNB REMACO O&M SPG immediately, report need endorsed by
qualification person (agreed by TNB REMACO O&M SPG) and to submit within 24-hours from
the test.

5 INSPECTING AUTHORITIES
6.1 Vendor shall appoint for Inspecting Authorities for second interpretation of NDT results as per
Fourth Schedule FMA 1967 if requested by station. The price for each inspecting authorities
need to specify as Schedule A4.

6 WORK EXPERIENCE/S

7.1 The Contractor SHALL submit its work experience/s and qualification on similar works in TNB/SPG/IPP
plants/petrochemical plants/oil and gas plants in this tender for evaluation

9 GENERAL SAFETY COMPLIANCE

9.1. All contractors must comply to the SPG Safety Requirements and expectations.
9.2. To cordon off work area by using standard warning tape with standard safety signage prior to
commencing work to the satisfaction of the Station Management and/or Representative.
9.3. Access openings and holes in the ground or floors shall be fenced and cautioned with hard
barricade and with sufficient standard safety signage.
9.4. To ensure all workers wear safety boots and safety helmet at all times. Other PPE such as face
mask and goggle are also mandatory for certain activities as identified in the JSA/HIRADC.
9.5. To ensure all workers wear ear plug/mufflers inside the station designated areas.
9.6. To ensure all workers wear safety body harnesses and any other protective safety gear when
working at level 3 meters or above ground on a platform.
9.7. All vehicles to be park at designated areas only.
9.8. Any work involving confined space by Contractor shall follows Industry Code of Practice for Safe
Working in A Confined Space 2010 and any superior code of practices by DOSH.
9.9. HOUSEKEEPING MUST BE CONTINUOUSLY CARRIED OUT THROUGHOUT THE WORKS AND
FINAL HOUSEKEEPING MUST BE AFTER THE WORKS COMPLETED AND THE WORK SITE MUST
BE RESTORED TO ITS ORIGINAL STATE PRIOR TO THE WORK COMMENCEMENT.

10 GENERAL CONDITION OF CONTRACT (GCC)

10.1 General Condition of Contract will be employed is GCC for Services TNB REMACO O&M SPG.

11 COMPULSORY DOCUMENTS FOR TENDER SUBMISSION (MANDATORY)

11.1 Contractors shall ensure that all schedule of prices are completely filled. Incomplete schedule may
result in contractor being disqualified.
11.2 The Contractor must attach / complete these documents with the tender submission:
11.2.1 Schedule A – The Contractor’s work experience & qualification in similar works in TNB/SPG/IPP
plant. Please attach copies of PO/reports etc.

11.2.2 Schedule B: CV for Engineers & Supervisor and their experiences related to such of work
(Inspection, Assessment and Non – Destructive Testing (NDT) works for Boiler and Unfired
Pressure Vessel) Experience at TNB, IPPs Coal-Fired Power Plant (300 MW/Unit and above) IPP
plants/petrochemical plants/oil and gas plants in Malaysia for similar work
11.2.3 Schedule C – Copies of company’s profile and NDT procedure (DPT, PAUT, RT, UT, UT Flaw
Detection & hardness Test) certified by NDT level III for each NDT. Procedure shall be owned
by the contractor company).
11.2.4 Schedule D: Detail of manpower list
