Schedule for Document Verification, Counselling & Subsequent Joining

In continuation of Public Notice-2; Dated: 02/05/2022 and, Public Notice-1; Dated: 24/04/2022 through
which Result of the OMR Based Exam held on 24/04/2022 was declared on NHM Website.
A. Now District-wise & Category-wise Provisional Merit List has been prepared on the basis of marks
obtained by the candidates in the OMR Based Exam held on 24/04/2022 + Additional 5 marks
considered on account of Six Months BPCCHN Course from IGNOU/University/Institution assigned
by the State, or MLHP Course integrated in the Curriculum of BSc Nursing passed after Academic
Session2019, as per the GoI/MoHFW Notifications.
B. Accordingly, “District & Roll Number-wise Combined List of Provisionally Shortlisted Candidates
prepared in order of above referred merit is given in Table-1; wherein Date and Time for document
verification is mentioned before name of each Candidate (Click here for Table-1).
C. Therefore, the Provisionally Shortlisted Candidates as per Table-1 are required to appear
before the Scrutiny Committee for Physical Verification of their Original Documents (i.e.
Qualifying Academic Certificates (including Six Months Mid Level Health Provider’s Bridge Course
Programme on Certificate in Community Health for Nurses (BPCCHN) from IGNOU or any
University/Institution assigned by the State, or MLHP Certificated integrated in the curriculum of
BSc Nursing passed after 2019 Academic Session) and Valid Documents issued by Competent
Authorities for their reservation claims or any other benefit(s) under various categories), and
subsequent Counselling, as per the below given Schedule and Venue:

Note: If any Candidate, produce fake document(s) in terms of academic qualifications, Certificate(s) for

Course, or claiming Reservation Benefits etc. will be barred from future Exam(s) of NHM & Health
Department (Haryana), and even criminal action may also be taken against such Candidate(s).

A. Detailed Instructions for the Candidates regarding Document Verification:
It is reiterated that, only Provisionally Shortlisted Candidates in order of merit, whose Roll No. are
mentioned in Table-1, are required to appear for physical verification of their Original Documents ((i.e.
Qualifying Academic Certificates (including Six Months Mid-Level Health Provider’s Bridge Course
Programme on Certificate in Community Health for Nurses (BPCCHN) from IGNOU or any
University/Institution assigned by the State, or MLHP Certificated integrated in the curriculum of BSc
Nursing passed after 2019 Academic Session) and Valid Documents issued by the Competent Authorities
for their reservation claims or any other benefit(s) under various categories).
1. All Shortlisted Candidates are required to bring printed copy of their online filled Application Form
and Admit Card/Roll No. with Identity Proof for appearing Document Verification.
2. All Provisionally Shortlisted Candidates are required to bring Self- Attested 5 Sets of
Photocopies of their all supportive documents, and 5 latest Passport Size Photos
3. The Provisionally Shortlisted Candidates belonging to all categories are required to produce all
“Original Documents” in support of their candidature, as per detail given below:
3.1 Additional 5 marks will be considered subject to producing the Six Months Mid-Level Health
Provider’s Bridge Course Programme on Certificate in Community Health for Nurses (BPCCHN)
issued by IGNOU or any University/Institution assigned by the State, or MLHP Certificated
integrated in the curriculum of BSc Nursing passed after 2019 Academic Session issued by
concerned Nursing College, as per the details given in the advertisement dated 20/03/2022under
Para-2 about Essential Academic Qualifications. No other document(s) will be considered for
providing additional 5 marks for this purpose.
3.2 All Provisionally Shortlisted Candidates for Scheduled Castes Category belonging to Haryana
are required to produce their Original Scheduled Caste Certificate alongwith Haryana Domicile
Certificate issued by the Competent Authority. Further, all these Candidates are also required
to produce a Self-Declaration/Undertaking declaring her/his “Eligibility for Scheduled Caste”.
3.3 All Provisionally Shortlisted Candidates for Backward Class (A or B) Categories belonging to
Haryana are required to produce their Backward Class Certificates (clearly indicating A or B
category) alongwith Haryana Domicile Certificate issued by the Competent Authority. All these
Candidates are also required to produce a Self-Declaration/Undertaking declaring her/his
“Eligibility for Backward Class (A or B)”. The OBC Certificate shall not be considered for benefits
of reservation under BC-A or BC-B Category. Further, all these Candidates are also required to
produce a Self-Declaration/Undertaking declaring her/his “Eligibility for Backward Class (A or B)”.
3.4 All Provisionally Shortlisted Candidates for Ex-Servicemen/Dependent Categories belonging to
Haryana are required to produce their latest Ex-Service Men/Dependent Certificates alongwith
Eligibility Certificate issued by the Competent Authority, which should not be more than one year
old. All these Candidates are also required to produce an Affidavit declaring her/his “Eligibility as
Ex-Service Men/Dependent of ESM”. All reserved category candidates under ESM/Dependent
(Except ESM/Gen) are also required to produce Haryana Domicile Certificates for claiming their
reservation benefit. Further, all Candidates are also required to produce a Self-Declaration/
Undertaking declaring her/his “Eligibility for Ex-Servicemen / Dependent category.
3.5 All Provisionally Shortlisted Candidates for Economically Weaker Sections u n d e r General
Category (EWS-GEN) belonging to Haryana are required to produce their latest “Economically
Weaker Sections Certificate with income limit of Rs. less than Rs. 6.00 lakh (which should not
be more than one year old) alongwith their Haryana Domicile Certificate issued by the
Competent Authority. All these Candidates are also required to produce a Self-Declaration/
Undertaking declaring her/his “Eligibility for the EWS (General Category) belonging to Haryana”.
If the quota reserved for Ex-servicemen remains unfilled due to non-availability of suitable Ex-
servicemen or their dependents, than chance will be given to the Freedom Fighters’ Children/
Grand Children upto the extent of available reserved seat(s) under ESM category.
3.6 All Provisionally Shortlisted Candidates for Outstanding Sports Person Category belonging
to Haryana are required to produce their “Outstanding Sports Person Certificates” alongwith
Haryana Domicile Certificate issued by the Competent Authority. All these Candidates are
also required to produce a Self-Declaration/Undertaking declaring her/his “Eligibility for
Outstanding Sports Person Category belonging to Haryana”.
3.7 In case of equal marks obtained in Written Exam, the merit to be decided on the basis of seniority
in age, and subsequently seniority in passing qualifying degree (if further required).
3.8 If any candidate is not able to produce any relevant document(s) pertaining to her/his academic
qualification(s)/category Certificates etc. at the time of document verification, her/his candidature
will be finalized according to the produced document only and no other chance will be given to any
of the candidate for reproducing any supporting document away from the fixed schedule.
3.9 Final Selection will be subject to the Document Verification and District & Category-wise merit
finalized on the basis of marks obtained in OMR Based Exam + 05 Additional Marks to be given to
the candidates, who have already completed the BPCCHN, or MLHPs course integrated in the
curriculum of BSc Nursing/Post-Basic BSc Nursing; for which original certificates shall require to
be produced to ensure their eligibility in terms of academic qualifications, BPCCHN/MLHPs course,
age, reservation benefit (if any), Haryana Domicile etc.. It is further clarified that, the Candidates
mentioned in Table-1 have only been provisionally shortlisted and not selected for the said post.

 3.10 It is reiterated that appearing for the document Verification shall not be considered as selected for
the said post, as the final selection is subject to the fulfilment of prescribed eligibility criteria and
verification of original documents, as per the advertisement. The final selection of these
provisionally shortlisted candidates will depend upon the following:
(i) No. of post(s) advertised in particular District & Category,
(ii) Her/his rank as per Written Test Merit + Inclusion of additional 5 marks as per detail given
under Para 3.1 and in the
(iii) Reservations benefits subject to producing the relevant supporting document(s) issued by
the competent Authorities.
(iv) Meeting the overall eligibility criteria, as per advertisement.
3.11 Final Result will be prepared and declared after completion of document verifications subject
to conditions given above in this Notice. The finally selected candidates on the basis of
document verification shall have to attend Counselling at NHM SHQ as per the schedule for
final joining by way of signing their Surety Bond with NHM, and Contract with the O/o
respective Civil Surgeon-cum-Executive Secretary, District Health & Family Welfare Societies.
3.12 It is reiterated that if any Candidate, produce fake document(s) in terms of academic
qualifications, Certificate(s) for claiming additional 5 marks on account of the said Six Months
BPCCHN Course or Integrated MLHP Course, or claiming Reservation Benefits etc. will be
barred from future Exam(s) of NHM & Health Department (Haryana), and even criminal
action may also be taken against such Candidate(s).
B. Detailed Instructions for the Candidates regarding Counselling and Subsequent Joining :
The final Result will be declared after document verification for which Public Notice will be uploaded at
NHM Website, and final selected Candidates are required to attend the Counselling Session at NHM SHQ
as per the above given schedule & Venue. During Counselling, following documents are required to bring
by the Candidates:
(i) Rs. 100/- ‘blank’ Affidavit for singing of the Surety Bond: The Surety Bond will be printed
by NHM SHQ during Counselling.
(ii) Rs. 10/- ‘blank’ Affidavit for singing of the Contract with the O/o respective Civil Surgeon-
cum-Executive Secretary, District Health & Family Welfare Societies.
(iii) Valid Identify Card for entry in the Counselling Session.
(iv) All Candidates are required to adhere to the Schedule and Reporting Time, as no further
chance will be given to any of the candidate for Counselling.
1. After signing of the said Surety Bond and Contract, the Candidates, who have done the Six Months
Mid-Level Health Provider’s Bridge Course Programme on Certificate in Community Health for
Nurses (BPCCHN) from IGNOU or any University/Institution, or who have completed the MLHP
Certificated integrated in the curriculum of BSc Nursing passed after 2019 Academic Session, will
be given direct joining as MLHPs-cum-CHOs, and remaining Candidates will be considered for 6
month BPCCHN Course from IGNOU in the forthcoming batch as per the Academic Session of
IGNOU. This all will also be in order of merit and advertised District & Category-wise seats.
2. The MLHP-cum-CHOs already working under NHM Haryana are abide for three years’ Service
Bond as per existing norms and all such candidates are required to submit their undertaking for
the same at the time of Document Verification. In case of concealing the facts by any of the existing
CHO, administrative and legal action will be taken against him/her.
3. No separate communication will be sent to any of the Candidate regarding Physical
Verification of the Documents or Counselling.
4. No TA/DA will be given to candidates appearing for the document verification & Counselling.
5. All communication will be issued/uploaded only through/on NHM Haryana Official Website (i.e.
www.nhmharyana.gov.in). Therefore, all candidates are suggested to keep checking the NHM
Website for latest updates.
Therefore, all provisionally Shortlisted Candidates as per the ‘ Table-1’ are required to appear for
Document Verification before the Scrutiny Committee as per the Schedule & Venue alongwith their
all Original Certificates. Further, selected Candidates as per the final result to be declared after document
verification are required to attend the Counselling for signing of the Surety Bond & subsequent joining,
and failing which their candidature will not be considered for final selection. No other date (except
mentioned above) will be given to any of the Candidate for Document Verification or Counselling, which
is required to be adhered by all the Candidates.
Note: (i) In view of COVID 19 pandemic, only the Candidates will be allowed to enter in the Venue, and all the
Candidates should wear masks, maintain social distancing and to adhere the COVID-19 Guidelines,
while coming for their document verification.
(ii) All Candidates are required to adhere to the Schedule and Reporting Time, as no further chance will
be given to any of the candidate for document verification and Counselling.
(iii) Table-1 is uploaded o the link at Sr. No. B at Page 1 of 3.
