VALLESTEROS Chief Administrative Officer Philippine Statistics Authority Centris Eton One, Quezon Avenue Mandaluyong City Dear Ms. Vallesteros: This refers to your request for the conduct of a Career Service Examination — Pen and Paper Test (CSE-PPT) by the Civil Service Commission — National Capital Region (CSC-NCR) on December 12, 2021 Initially, you have submitted to the Examination Recruitment and Placement Office (ERPO), Civil Service Commission Central Office (CSC-CO), the list of your employees who signified to take the ‘said examination. The list has already been uploaded in the Online Career Service Examination Application System (OCSEAS). In this connection, please be guided by the following guidelines in the registration and acceptance of application forms: 1. Deadline for online registration via link httpsi//iwapv2.cscro12.com is on November 24, 2021 (Sunday), 11:69 p.m. No extension willbe allowed, 2. Deadline for the submission to CSC-NCR of the hard copy of the application forms, including the required registration fee of Php500.00. is on November 22, 2021 (Monday), 5:00 p.m. Only those submitted on the said date will be evaluated and processed Application forms submitted beyond November 22, 2021 will be disapproved. 3. Applicants shall be limited to bonafide employeesijob orders of your agency. Ride-on arrangement for outsiders not belonging to the requesting agency is strictly not allowed, including children and/or other relatives of the agency employees/workers. 4. Applicants shall not be allowed to change the level of examination (Professional/Subprofessional) originally indicated at the time they signified their intentions to take the examination 5. Strictly, individuals not included in the agency list of examinees shall not be accepted. Replacement or additional applicants shall not be permitted once the list is downloaded in the OCSEAS. 6. After successful online registration, individual applicants shall do the folowing: a. print their OCSEAS generated application form in a legal size bond paper; b. paste 2 of their original 1.. photos on appropriate spaces provided on the application form: ©. place the remaining 2 original I.D. photos inside a transparent pouch, and attach/staple the pouch on the upper left corner of the application form's printout; Bawat Kawani, Lingkod Bayani ‘F No. 25 Kaliraya St, Brgy. Card with indicated date of birth and validated by the agency focal person. For outsourced workers without agency-issued Office I.D. card, their (private) Company issued |.D. card may be presented/submitted, together with a certification from the agency HR that they are currently under service contractjob order with the requesting government agency, f. attach a photocopy of other valid ID Card if Office/Company I.D. does not indicate any date of birth; g. attach a duly accomplished original copy of the Certificate of Consent; h. attach a photocopy of the Vaccination Card; and i. submit all documentary requirements to the Agency Focal Person, together with the exam fee. 7. The Agency Focal Person shall do the following ‘a. accomplish the “jurat” portion on each application form; b. check the completeness of the application documents; ¢. collect the exam fees; and 4d. prepare the list of examinees based on submitted application form printouts, and {group the list according to vaccinated and unvaccinated examinees, together with the indorsementitransmittal letter 8. Additional applications requirements like the RT-PCR/Saliva Test or Antigen test will be discussed in a separate communication, Please be guided accordingly. Thank you.
