This is the Gratuity Settlement Form. You are eligible to fill this form if you had been associated with us for a minim
20 days after the forms reach the Corporate Office as the payment is to be made by Life Insurance Corporation(LIC

This form should be submitted in case you are a member. You may transfer your Superannuation Balance to the C
obtained from that Company. In case you would like to commute your Superannuation balance, 30% of the accumu
gratuity and 50% in case you are not eligible for gratuity. The commuted value would be paid to you only after dedu
amount in your account will be paid to you as pension monthly, quarterly or half yearly as you may decide. Any suc

What happens when you leave HPI?

When you leave HPI, your participation in the SOP ends. HPI will refund any accumulated contributions for the curr

MIS will continue to hold your shares in your account. You can continue to access this account in the same manner

(i) If you continue to hold your shares in your MIS account.
You will continue to have your dividends reinvested into additional shares, except in countries where reinvestment o
receive a dividend check. You will receive a year-end statement in the first month of each year confirming your hold

(ii) If you choose to sell shares through MIS:


The timing will be the same as for all Share Ownership Plan participants. Funds will be disbursed on the settlement
currencies, which take two additional days.

(iii) If you elect to request that MIS transfer your shares to Computer share Investor Services:
Please allow approximately two weeks for the shares to be transferred to Computer share. You should receive a pr
