There are 2 simple ways to take advantage of the program:

1. Online –
a. Enroll in the employee savings program by visiting att.com/teamhealth
 Account must be in the employees name
 Follow the onscreen prompts
 Enter your personal email address and the primary line on your AT&T Wireless account when required
Please allow 1 to 2 bill cycles for the savings program to appear on your statement.

2. In person – visit your local AT&T store; to find the store closest to you visit att.com/find-a-store.
a. Enroll in the partner savings program
 Provide proof of employment (id badge, current paystub, work email address)

b. Sign up for new service
 Switching to AT&T – recommend that you bring your wireless statement so the port process goes
smoothly
