Instructions: The questions below relate to factors most closely associated with the capacity to
cope successfully with stress. The items will ask about wellness, thought-control, active coping,
social ease, tension reduction and spiritual practice. Circle the letter which lists the option that
you choose. Answer each question as honestly as possible.

two drinks within a short period such as an evening.)
