CONTRACT OF EMPLOYMENT

Position : Drug Analyst
Department : Laboratory Department

This is to inform you that PARAÑAQUE ULTRASOUND DIAGNOSTIC CENTER, INC. has
employed you fulltime on REGULAR STATUS under the following terms and conditions.

1. You shall hold the position of Drug Analyst of Parañaque Ultrasound Diagnostic Center,
Inc. (PUDCI).

2. Your employment takes effect on March 02, 2015.

3. You shall receive an incentive of P5,000.00 per month for the drug testing patient
(referred or walk-in) of the company. This incentive will be given to you every end of the
month.

4. The clinic expects that you will diligently perform your responsibility and will be compliant
with the rules and regulation as stated in the Dangerous Drug Act of 2002 and the and
the guidelines of the Department of Health.

5. You are expected to carry out and perform your duties and responsibilities to the best of
your knowledge and ability, abide by the Standard Operating Procedures and the
Policies, Rules and Regulations –PUDCI Code of Conduct. Corresponding disciplinary
actions will be imposed for any infractions or violations thereof.

You are to uphold PUDCI’s Mission and Vision, and expected to actively participate in
meeting its Quality Objectives and help maintain its high standards of quality service.

6. This employment contract may be terminated at any time during your employment due
to: inefficiency, unsatisfactory work performance, violation of company rules and
regulations, derogatory information revealed in the course of your background check,
false statements, pending criminal charges in your police and NBI clearance reports, and
such other causes considered by the company as a disqualification for your continued
employment.
7. Should you resign at anytime during your employment, you shall give the company a
written notice at least thirty (30) days before the date of effectivity to give ample time for
management to recruit and train your replacement. You are to accomplish an exit
clearance a week before date of effectivity as a prerequisite in getting whatever
remunerations still due you or requests for any company certification
