I. OBJECTIVES:
 Enrollment of Grade 7- Dandelion to Grade 8- Amitrine.
 E-BASA
 Reading Assessment in English.
 Learners Enrollment Form

II. PARTICIPANTS:
 Enrolling Teacher ( Edessa D. Masinas)
 G7- Dandelion Students
 Parents/Guardians.

III. HIGHLIGHTS OF THE ACTIVITIES:

The Enrolling Teacher, Ms. Edessa D. Masinas. Starts the enrollment at exactly 8:00
A.M. thru home visitation of each student’s houses at Purok 3, 4, & 5 Brgy. Canlapwas,
Catbalogan City, Samar.

As a requirement for enrollment, each students are required to submit PSA/NSO and their
SF 9, some of students PSA/NSO can’t submit due some concerned such as: Guardian
doesn’t know where the parents put their students PSA/NSO and Some are needed to use
for some other matter that needs the students PSA/NSO while some students cannot
enrolled due to lack of PSA/NSO.

The Enrolling Teacher let the Parents/Guardians to fill up the Learners Enrollment Form
while their students were being evaluated in Reading Assessment in English, E-BASA in
Filipino and Numeracy Assessment in Math.

IV. PROBLEM/S MET:

 Guardian doesn’t know where the parents put their students PSA/NSO
 Some are needed to use for some other matter that needs the students PSA/NSO.
 Some students cannot enrolled due to lack of PSA/NSO.
V. SOUTIONS/ RECOMMENDATIONS:
 PSA/NSO can be followed till last day of enrollment.
