JOINT-AFFIDAVIT

We, CORNELIO G. TAMON, 64 years old, single, and

ESMERALDA B. OCAMPO, 47 years old, married, Filipinos, and
residents of Brgy. Buyu-an, Tigbauan, Iloilo, Philippines, after being duly
sworn to in accordance with law, hereby depose and say the following:

That we personally know MERLINA BALDEDARA GERMINAL

who is the wife of ELISEO G. GERMINAL and the mother of MA.
ELAINE BALDEDARA GERMINAL who was born on May 14, 2001 at
RPGTMH, Guimbal, Iloilo;

That in the Certificate of Live Birth of MA. ELAINE BALDEDARA

GERMINAL issued by the Philippine Statistics Office, her mother’s name
entered therein is MERLYN BALDEDARA instead of MERLINA
BALDEDARA;

That MERLYN BALDEDARA, the name appearing in the

Certificate of Live Birth of MA. ELAINE BALDEDARA GERMINAL
issued by the Philippine Statistics Authority, and MERLINA
BALDEDARA GERMINAL, the name appearing in the latter’s Certificate
of Marriage refer to one and the same person;

That we know these facts because we are close friends and neighbors
to her family;

That we are executing this affidavit to prove the truth of the foregoing
statements;
