How To Get A Name

Change in Kentucky
Step 1: Fill out and print the following forms. They
all need to be typed, not handwritten.

Check the box that says you don't have an attorney and
check off that it's a name change. The top right box needs
to have a county listed.

The form above needs to be notarized. UPS can notarize it
for you for $13.00, but you can maybe get it done at your
bank for free.

Make sure you fill out 1-4.

Step 2: Bring all of the forms from Step 1 to the


Circuit Court (Probate Dept. in Jefferson and Fayette
County)
The cost for filing is
$75.50 (You pay this at the Probate Circuit Courts
Office when you drop off your forms)
$5.00 for each certified copy and $0.50 for each
attested copy.

You need a certified copy to update your passport and


social security card, but you can use an attested copy to
update your driver's license.
Step 3: A Circuit Court judge will review your name
change request.
In Jefferson County, you are not required to go in front of a
judge to have your name changed. However, in some
counties, you may have to appear in court.

Step 4: Court Date (If required in your county)


When you arrive at your court date, the judge will ask you
some questions to confirm your information and then give you
a decision. If you want more information on how this process
works in your county, you can call your circuit court or email
us at transhealth@khjn.org, and we can call and ask for you.

Step 5: In some counties, you will get an invoice in the


mail to pay the County Clerk's Office.
This fee is $46. This seems to be a new step they say is the
fee for them to record the name change
Step 6: Change Your Name with The SSA
To update your name on your social security card, you will
need to bring the following to your local Social Security
Office:
a certified copy of your name change order
current social security card
a state-issued ID
They will update your name in their system and send you a
new Social Security card via mail.

Step 7: 24 hours after you have updated your name


with the SSA you can get an updated driver's
license at your local Drivers Licensing Regional
Office. You have to do this within ten days.

You should bring a copy of your name change order


(certified or attested)
The cost will be $15 unless you are within six months of your
expiration date, if you are, it’ll be $21.50 for a standard
license or $24 for a REAL ID.
