KNOW ALL MEN BY THESE PRESENTS:

THAT WE, Quezon City Government, a chartered city corporation duly created and existing under and by virtue of
the laws of the Republic of the Philippines, represented in this act by _________________________, School Principal of
SCHOOLNAME hereinafter referred to as the DONOR;

Name of the Student, Grade Level and Section, a student of SCHOOLNAME, with home address at
________________________________________ , herein referred to as DONEE.

WITNESSETH

That the DONOR is the owner of the total qty per batch units of SAMSUNG TABLET and its accessories procured
thru the initiative of Honorable Ma. Josefina G. Belmonte, City Mayor, and executed thru Ordinance No. SP 2954, s.2020;

That said SAMSUNG TABLET and its accessories were distributed and issued to learners of SCHOOLNAME, this
City;

That one unit of SAMSUNG TABLET and its accessories with IMEI No. ________________ and Property No.
________________________ is issued to STUDENT NAME as per attached copy of KASUNDUAN dated
;

That the DONEE is a verified graduate student of SCHOOLNAME, Quezon City for School Year 202_ - 202__.

That for and in consideration of a desire to contribute a share to the distance learning modalities in education and
provide incentives for all graduate students of Grade 6, Grade 10 and Grade 12 of SCHOOLNAME, and as an act of
gratitude and liberality on her Part, the DONOR, pursuant to Resolution No. ________ re: “ A RESOLUTION
__________________________________________, subject tablets are already conveyed gratuitously to the recipient.

That the DONEE hereby accepts and receives this Donation made by the DONOR with the attached
acknowledgement receipt and hereby manifests his/her gratefulness for the latter’s generosity and liberality.

That this DEED of DONATION is made and executed in Quezon City, Philippines on this ____ day of _______,
