BRIEF CASE SUMMARY:

A 56 year old female with no history of comorbidities presented to RGCI opd with complaints of lump in left breast since 6 months.

mammogram (8/2/22)- BIRADS 5 lesion left breast.FNAC- ductal carcinoma .

She came to RGCI for further management.

On examination, a 5x5 cm lump was palpable in upper inner quadrant of left breast (cT2N0).she was further investigated in RGCI.

Biopsy from left breast lesion- invasive ductal carcinoma, ER- 90%, PR- 40%, her 2 neu - negative.

PET CT SCAN- metabolically active lesion left breast, with ipsilateral lymph nodes. no evidence of metastasis.

She underwent LEFT MODIFIED RADICAL MASTECTOMY UNDER GA ON 19.02.2022.. e.

OPERATIVE FINDING:

A 5X5 cm LUMP SEEN IN THE LEFT BREAST IN THE UPPER OUTER QUADRANT OF THE BREAST. MILD ENLARGED LYMPH NODES IN THE LEFT AXILLA.

HPE--Invasive breast Carcinoma pT3NoMo, ER/-90%, PR-40%, Her -ve.

She receive adjuvant Chemotherapy with 4 AC and 4 T last on 29th June 2022.

She tolerated the treatment well.

Now planned for RT.

Patient to be examined on next visit.

IMPRESSION -- Ca left Breast Post MRM, Post Chemo Er+ve, PR +ve. Her -ve

Plan -- adjuvant RT to the chest wall and SCL LN with DIBH.

all pros and cons of Rt explained to the patient and her son, including discolouration of skin. Mild dysphagia, Decreased Blood counts.

PEt CT on 20th June.
