**PARENTAL CONSENT**

We, the undersigned hereby willingly and voluntarily give consent

for the participation of our child/children in the implementation of
the limited Face-to-Face classes at Magalalag National High School
for S.Y. 2022-2023.

We have considered the benefits our children will derive from their
participation in the said face-to-face classes provided that due care
and protection will be ensured, and that the school will assure to
strictly follow the required health protocols and standards set by
the COVID-19 Inter-Agency Task Force (IATF).
