2. Purpose of home visit?
To ask a copy of student’s Birth certificate and also give important updates about school matters
3. Who was present?
Krisyl’s mother and sisters
4. Are the family’s basic needs being met (i.e. electricity, heat, hot water, adequate food and
clothing, etc.)?
Unfortunately, the basics were not able to meet by her family. By physically looking at the
status and current situation they have, it is evident that her family lacks enough food and
electricity.
5. What issues were discussed at the home visit?
The issues discussed during the home visit are the following:
 Krisyl’s performance particularly the scores she got in her Modules.
 Observing the correct timeline in answering and submitting her Modules
 Incomplete forms needed by the adviser
 Student’s family status
6. Were any recommendations/suggestions given to parent/family? Yes
      a) What were they?
The following advices were given to the parents and student:
 Encourage positive motivational learning style/process to continue learning
 Provide opportunities to improve learning and close the gap between current
situation and educational goal
 Helps student develop self- assessment
 b) Who is responsible for follow up?
It should be Krisyl and her parents
7. Next steps?
I provided a form to the students which will be her basis in doing self-assessment of her
work so that she can identify her strengths and weaknesses along with her work submission.
