‘The Prosecuting Attomey for Kanawha County upon his information and belief charges:

‘one year of filing this information, in the County of Kanawha, West Virginia, Lisa Perdue, while
That on the 8th day of September, 2021, and within
working as a classroom aide at Holz Elementary School, and being a mandatory reporter as defined
in West Virginia Code § 49-2-803(a), and having reasonable cause to suspect a child was being
abused and neglected and that the said Lisa Perdue observed the child being subjected to conditions
that were likely to result in abuse and neglect, failed to within 24 hours report the circumstances
of the abuse and neglect to the Department of Health and Human Resources, to-wit: observed
classroom teacher, Nancy Boggs, grabbing and jerking A.S., a child of nine years of age, without
reporting the incident to the Department of Health and Human Resources within 24 hours, against
the peace and dignity of the State of West Virginia, in violation of Chapter 49, Article 2, Section
803(a), of the Code of West Virginia, 1931, as amended.

abused and neglected and that the said Lisa Perdue observed the child being subjected to conditions
that were likely to result in abuse and neglect, failed to within 24 hours report the circumstances
of the abuse and neglect to the Department of Health and Human Resources, to-wit: did observe
the classroom teacher, Nancy Boggs, isolating S.P., a child of seven years of age, in a bathroom
with the lights off, without reporting the incident to the Department of Health and Human
Resources within 24 hours, against the peace and dignity of the State of West Virginia, in violation
of Chapter 49, Article 2, Section 803(a), of the Code of West Virginia, 1931, as amended.
22M. 1% (COUNT FOUR: That on the 22nd day of September, 2021, and within
one year of filing this information, in the County of Kanawha, West Virginia, Lisa Perdue, while
working as a classroom aide at Holz Elementary School, and being a mandatory reporter as defined
in West Virginia Code § 49-2-803(a), and having reasonable cause to suspect a child was beingabused and neglected and that the said Lisa Perdue observed the child being subjected to conditions
that were likely to result in abuse and neglect, failed to within 24 hours report the circumstances
of the abuse and neglect to the Department of Health and Human Resources, to-wit: did observe
the classroom teacher, Nancy Boggs, repeatedly verbally abusing A.S., a non-communicative child
of nine years of age, without reporting the incident to the Department of Health and Human
Resources within 24 hours, against the peace and dignity of the State of West Virginia, in violation
of Chapter 49, Article 2, Section 803(a), of the Code of West Virginia, 1931, as amended.

‘one year of filing this information, in the County of Kanawha, West Virginia, Lisa Perdue, while
‘working as a classroom aide at Holz Elementary School, and being a mandatory reporter as defined
in West Virginia Code § 49-2-803(a), and having reasonable cause to suspect a child was being
abused and neglected and that the said Lisa Perdue observed the child being subjected to conditions
that were likely to result in abuse and neglect, failed to within 24 hours report the circumstances
of the abuse and neglect to the Department of Health and Human Resources, to-wit: did observe
the classroom teacher, Nancy Boggs, isolate S.P., a child of seven (7) years of age, behind a screen
for four (4) hours without food, water, and access to the restroom, without reporting the incident
to the Department of Health and Human Resources within 24 hours, against the peace and dignity
of the State of West Virginia, in violation of Chapter 49, Article 2, Section 803(a), of the Code of
West Virginia, 1931, as amended.
22m. 1% (COUNT SIX: Thaton the 22nd day of September, 2021, and within one
year of filing this information, in the County of Kanawha, West Virginia, Lisa Perdue, while
‘working as a classroom aide at Holz Elementary School, and being a mandatory reporter as defined
in West Virginia Code § 49-2-803(a), and having reasonable cause to suspect a child was beingabused and neglected and that the said Lisa Perdue observed the child being subjected to conditions
that were likely to result in abuse and neglect, failed to within 24 hours report the circumstances
of the abuse and neglect to the Department of Health and Human Resources, to-wit: did observe
the classroom teacher, Nancy Boggs, shove A.S., a child of nine years of age, with foree into a
table, without reporting the incident to the Department of Health and Human Resources within 24
hours, against the peace and dignity of the State of West Virginia, in violation of Chapter 49,
Article 2, Section 803(a), of the Code of West Virginia, 1931, as amended,
