    I, the undersigned, Gloria N. Bartolome, Filipino, mother of Ms. Hershey N. Bartolome, 17 years
    old, minor age, presently residing at #84 Kamagong St., Metroville, Rosario, Santiago City. I
    hereby authorize Ms. Jessamae N. Jacinto to and on my behalf in all manners relating to all
    dealings with the Department of Foreign Affairs for the renewal of my daughter’s passport,
    including signing of all documents relating to these matters. Any and all acts carried out by Ms.
    Jessamae N. Jacinto, on my behalf shall have the same effects as on my own.

    I am hoping for your kind consideration regarding this matter.
