Dear Ms. Sonen ,

I hereby inform that I have been in the process of obtaining the Nuclear Medicine
License from the Texas Medical Board for over a year. During this time I have send all
the documentation on time and I have fulfilled the necessary requirements to obtain the
license. There has been a problem of severe communication with the person to whom my
case was assigned, Amy Elizondo.

From August 7 2017 Amy Elizondo sent me the requirements and the
documentation that I needed to send from the Puerto Rico Health Department. The Good
Standing was sent three times from the official office in Puerto Rico, first on September
12, October 26 and finally on November 10, 2017. I sent three emails asking for a
confirmation to know if she received the documentation from Puerto Rico. The first
message where I asked for a confirmation occurred in December 1, December 13 and
January 2. She answered me on January 25 2018 that she had received the
documentation. It should be noted that it took over two months.

The next time she contacted me was on March 15, indicating that the manager's
review had not been done yet. In April 24 I wrote an email to her telling her that I needed
an update. I received an email on May 3, almost a year later since I first started with the
application, where she informed me that I need 200 practice hours.

The Doctor in Nuclear Medicine Ricardo Santiago where I was planning to


perform my practice in Puerto Rico sent the documents signed on May 14. It should be
noted that Dr. Ricardo Santiago is currently the President of the Nuclear Medicine
Society in Puerto Rico.

I sent an email on June 22 to Amy Elizondo asking her if she received the
documentation from the doctor. She responded on June 29 (over 1 month later) that she
had never received the documents of the doctor in Nuclear Medicine Ricardo Santiago.
On August 6 2018, I received an email from my specialist indicating that the
doctor had to send all his licenses for the Texas Medical Board committee to review and
authorize my practice in Puerto Rico. On August 7 2018 Ricardo Santiago sent his
license (NRC) and the credentials of the laboratory. I called weekly as you can review in
my file to know the status of the documents I submitted.

I want you to know that the lack of communication resulted in misunderstandings,


incorrect messaging, incomplete directions and a host of other problems. Amy Elizondo
never told me that I needed to make my practice in Texas and that she was aware that
I live in Puerto Rico but I was planning to move to Texas.

I would like to clarify if I have until November 3 to submit the name of the
laboratory and the doctor who will supervise my practice in Texas or is by that date I
need to have completed my 200 hours and I have to start the process again.
