residing at Villa Dela Paz, San Miguel, Tarlac City, after having been duly
sworn in accordance with law, do hereby depose and state that:

1. That I am currently processing my employment papers with

PHILCANGCO INTERNATIONAL RECRUITMENT SERVICES;

2. That I have tried to register online but was unable to receive the
confirmation via electronic mail;

3. That considering the fact that it was my first time to process

employment abroad, I have failed to file my E-Registration and Pre-
Employment Orientation Seminar on time;

4. That I have no ill intention to delay nor commit any fraud in my

failure to secure the required registration;

5. That I further undertake that the said delay will no longer happen
again;

6. That I am executing this affidavit to attest to the truth of the foregoing

for whatever other legal purpose it may serve.
