SUBJECT : Mandatory Response to the Online Mental Health Questionnaire

In reference to CSC Memorandum Circular No. 04 series of 2020 on Mental

Health Program in the Public Sector and Republic Act no. 11036 on Mental Health, an
Online Mental Health Questionnaire shall be answered by all TESDA employees
through the link: https://forms.gle/DNiHuGziYVMtT5Js9 on or before June 23, 2021.

The questionnaire shall measure the well-being of each personnel measuring th^
stress, anxiety and depression level of the individual. The Human Resource
Management Division - Administrative Service (HRMD - AS) - TDI shall monitor the data
collected and answer queries on the abovementioned questionnaire.

This is to ensure and promote over-all mental wellness of TESDA personnel and
provide appropriate supportive Health and Wellness intervention Programs while
maintaining an inclusive, conducive and productive work environment.
