PRE EMPLOYMENT Medical PROCESS

A. PreEmployment Medical Examination at QuestPhil Diagnostics:

1. Quest Phil Diagnostics welcomes all candidates from industry partners at the arrival time at the
clinic from Mondays to Fridays 8am-10am & 12nn-2pm & Saturday 8am-10am. No patient shall undergo
pre employment medical examination on the following parameters:
a. With Covid 19 symptoms or similar indicators provided by DOH/WHO/Quest
b. no endorsement from the Company
c. presentation of valid gov’t ID with picture
d. no pea/monggo stool specimen
e. printed referral form (provided by the Company)
Quest to provide an explanation (for their safety during pandemic, candidates/patients should
only stay in the clinic for a limited time) and instruction (they may use the restroom on the 2nd
floor beside the Appliance Center at Robinsons Mall and return with their stool specimen.
2. Pink form format shall be: OK and affixed by duly authorized QuestPhil medical frontliners and
shall be submitted by the patient to Nurses once completed.
3. Talent Team or Nurses to verify the pink form with QuestPHil Team if with erroneous/
suspectable erasure/s.

B. Quest_Company Endorsement and Results Record

1. QuestPhil to send thru email patient results as soon as possible following the internal procedure
of releasing results
a. normal findings & fit to work classification releasing time is 4-5 PM Monday - Friday / Saturday:
Mon-Fri schedule
b. ASAP if requested by Talent Team
c. If not normal, with findings, for further study, with the recommendation, with the unsubmitted
specimen, results will be released as soon as resolved.
 2. Talent Team: to monitor and update everyday the ENDORSEMENTS sheet and send written
communications such as on group chat or email to inform QuestPhil for any concern, inquiries or missing
results.

C. PreEmployment medical Results

1. Cut off of premedical examination is every Mon-Fri 2pm and Saturday 10:00am.
2. Uploading of results for weekdays: every 4-5PM once validated by QuestPhil Team.
3. Premedical examination taken on Saturday: results shall be released on Monday of the
following week at 4-5PM.
4. For patients with Pneumonia/PTB and other significant findings tagged as pending: QuestPhil
still needs to upload the summary of results on the google sheet file and highlight it in red
ink. QuestPhil Team shall also inform the patient with pertinent instructions and next steps relevant to
his condition/completion of premedical examination.
