AFFIDAVIT OF WITNESS

I, REDENTOR MARQUEZ SAMBAAN, age, married, resident of Lapu-Lapu Village,

Poblacion, Sto. Nino, South Cotabato, after having sworn in accordance with the law,
do hereby depose and state THAT:

1. I was present and personally saw the vehicular accident involving DEXTER V.
LERON, Teacher I/Teacher In-Charge of Pulo Subong Integrated School- Fidaw
Extension.

2. The vehicular accident took place last May 28, 2021, at 4:00 p.m. along Sitio
Spartan, Brgy. Ned, Lake Sebu, South Cotabato. Mr. Leron suffered fractures in
his right leg, left arm, and shoulder.

3. This Affidavit is executed in order to attest to the truthfulness of the foregoing

narration of facts for the purpose of MR. Dexter Leron’s availment of
Rehabilitation Leave and for whatever legal purpose it may serve best.

IN WITNESS WHEREOF, I have hereunto set my hand this 2nd day of June 2021, in
