Congratulations!
Here are the procedures on the IN-PERSON ENROLMENT for Academic Year 2022-2023 for
your information and guidance. The IN-PERSON ENROLMENT PROCEDURE is intended for
the QUALIFIED FRESHMEN APPLICANTS. Please strictly follow the ENROLMENT DATE
assigned to you.
IMPORTANT: Please READ the COMPLETE steps BEFORE COMING to SLU for your
ENROLMENT. In addition, any misrepresentation /concealment of information will be
enough reason to disqualify/invalidate your enrolment in SLU.
For QUALIFIED BS in Nursing Applicants: Apart from the Basic Requirements required for all
Qualifiers, BSN Qualifiers will have to download, fill-up and prepare the following:
(1) Scanned copy of the duly signed School of Nursing Undertaking*
(2) Scanned copy of the medical certificate with complete laboratory results or a medical
certificate WAIVER*, and
(3) Photocopy of your Form 138. PLEASE WAIT FOR AN EMAIL CONFIRMATION BEFORE
PROCEEDING WITH YOUR IN-PERSON ENROLMENT. The forms can be downloaded from
your iSLU Portal , “Downloadable” Tab.
(Bishop Carlito Cenzon Sports Center formerly SLU Covered Courts, SLU Gate 2, Bonifacio St.)
1. Triage Process. Proceed to the triage area at the entrance gate of the Bishop Carlito
Cenzon Sports Center and present your completely filled-up Health Declaration Form
and Vaccination Card/Certificate. Only fully vaccinated students (Full 2 doses with
booster) will be allowed to enter the enrolment venue. If accompanied by a
parent/guardain, only 1 parent/guardian will be allowed subject to the submission of
the HDF and Vaccination Card (received 2 doses with booster).
3. Payment of Fees:
3.a Proceed to the Finance Office at the 2nd Floor Lobby, Diego Silang Building for
the:
I. Encoding of SLU ID Number (Window 6 or 7)
II. Payment of Corresponding Fees (Windows 9 to 12 )
3.c For SLU Employee Dependents proceed to the HRD Booth at the Enrollment Area
before proceeding to the Finance Office.
4. ID Processing. After payment of fees, proceed to the PE Building (near Enrolment
Hall) for Photo taking for Student ID.
5. Parent Portal. Parents/Guardians can register for an online access for the monitoring
of their child/ ward’s records at the UNIVERSITY REGISTRAR’S OFFICE (WINDOW 10,
Ground Floor Area.)
6. Claiming and Release of SLU IDs – Claiming of Student IDs will be on August 15 – 20,
2022 at Windows 5 and 6 of the University Registrar’s Office, Diego Silang Building.
