Subject: 221102- PAR TR14 Broken gear box housing.

1. Facts of the event:

2/11/2021, The operator informed that we found that the bolt of cardant shaft is broken as
tractor TR14 is carry containner.

2. Action taken:

We separateed trailer from tractor, and carry tractor to workshop prevent from traffic
jam .

3. Estimated of loss:
We need replace gear box housing (part connect to cardant shaft)
Replace oil and air hose, bolt of cardant shaft

4. Analysis reason of incident:

Bolts under heavy pressure load. It have tend lengthen and loosen due to fatique
material. As one of four is damaged leading not enough clamp force to hold connection of cardant
connetor firmly. With heay load on shaft, two part of cadant shaft is seperator. The operator is not
shutdown engine ontime ,rotating of free candant is touch to other part cause damaged hose and
broken housing

5. Lesson and preventive:

Need to check for replacement in time,.
