Description
On the above date and time I Insiders Coordinator Cooper was providing verbal council to y o u t ~ after a
group assault on another youth). During the session youth ■ disclosed several concerns where he expressed
that he believed several security set up theioup assault. He continued to express his concerns and share how
the events of the assaults unfolded. Youth also indicated that nothing was done to protect a n _
youth during the assault. I IC Cooper reminded youth ■ of the grievance process and expressed my full support
to him.

Description
On the above date and at ~oximate time, I was telephoned by Nurse Ashton Green asking if ii could bring
·uvenile #1111111111 to Birchwood Infirmary. I radioed Sgt. Roberta Williams to pull Juvenile
from the Pod for transportation. Upon my arrival t o ~ I was met in the vestibule by Sgt. Roberta
Wiliams. She stated to me that: "I don't think you wanna t a k e - - to the infirmary." I ask her why and she
replied: "Because he on the pod saying tha!l!,2.!.~..~~>n~ you back because they think you the one who set up
that situation last Thursday when him a n d - - ) got assaulted.

Prior to this (on the morning of the 6th) I had been confronted by Lt. Precious Kirkland after briefing who made the
statement to me that: "Favor you might want to be careful." I ask her why. She replied : 'Tm just saying, you might
be a target." I ask her what she meant but she gave no response.

I relayed the above information to my direct report: Jermaine Gordon, AFA and in turn he had me to also relay the
information to his direct report: Crayman Harvey. Their advise to me was: "just continue to take every threat as
serious and to watch my back at all times." I advised them that I would not feel comfortable intermingling with
juveniles on C-Pod at this time due the to tension over the situation. I further advised Mr. Harvey and Mr. Gordon
that I do have several concerns regarding possible staff contribution to the original assault on 06/02/2022 as well
as possible contribution to the contineing tension in the unit. Mr Harvey advised that we will schedule a peer
mediation between the juveniles in Holly and at that time also attempt the to resolve the notion of a hit on myself
or other staff.

Description
On the above date and at 15:15, this clinician was visiting the juveniles at Laurel, on the ITU side. As soon as
juvenile- (=tllllllll) saw him, he stated, "Mr. Stan, staff set us up." This clinician then said, "Walk me
throu h what happenedon Thursday evening." Juvenile- reported that Captain Favor called juvenile
out but does not know why. It may have been to get his tray, but don't know. Captain Favor asked juvenile
to stand by the door and that was w h e n - came out and started fighting with him. As they began
t a tain Favor gestured with his hand to let them fight and did not try to break the fight either." Juvenile
reported similar story and indicated that "the juveniles from A-POD were ready to fight juvenile
T ey must have known what was going to happen because they all had their shoes on and armed with
ro en room and mop sticks and other weapons in socks." - indicated that no staff intervened or tried to
break the fight. This clinician then asked juvenile- again."wfiere were the staff? . . then continued and
said "Man, I feel like the staff set us up. Captain Favor was right there and did nothing.' According to Juvenile-
- when he saw the juveniles from A pod armed with broomsticks and broken ~ s beaten on
Juvenile- "I went back to the pod and got a mop stick to try to help Juvenile-- When asked, if any
staff was there, he stated, yes, but none of them tried to intervene or stop the fight. Another juvenile who was also
involved with the fight, Juvenile ( , , . . ) indicated to this clinician around 1:30 pm in his office,
"Captain Favor c a l l e d - out to finish passing out the rest of the plates. It may have also been to go pick up
the clean clothes. I don't know. But all the kids in A-Pod had their shoes on and had sticks ready from the dust
mop. ~ r e ready to fight us. Someone from ~ s t u r e d ~ to come over the door and that's
w h e n - came out and started fighting w i t h - - O n c e ~ a n to f i g h t - - the rest of A-
Pod came out with their sticks and swings. Mr. Stan, I don't know. I know I went to the hospitalbk)ody."

Description
Multiple juveniles were involved in a physical conflict in the Holly Courtyard. Assistance was requested and PSO
Bates responded. PSO bates made attempts to defuse the situation but ultimately had to deploy chemical
munitions to control the situation. The situation was diffused and juveniles were taken for decontamination.

Description
On this day 6/2/2022 at approximately 13:45 pm, I Officer Joshua Heriott-Franklin walked around to each pod to
inform the juveniles I was leaving early. As I approached C Pod front door, it was unlocked by control staff. All
~ Pod proceeded to run out the pod into the courtyard where A Pod was. J u v e n i l e s - and
- - started to throw closed fist at each other and began to physically assault each other. I, JSII Franklin
gave directives for the Juveniles- and-ith to stop fighting. Assistance was called from Control
Room. All available staff arrived and situation ~eared .

On the above date and the approximate time, I Officer Amanda Wright was assigned to unit Holly control room. I
observed JCO Joshua Heriott-Franklin walk towards C Pod door. As JCO Heriott-Franklin approached C Pod door,
he pressed the button to enter. I unlocked C Pod door from within the control room . When I unlocked the door, all
juveniles from C Pod pushed the pod door o p ~ the courtyard where A Pod was waiting for their ride
to go to recreation at John G. Gym. J u v e n i l e s - - . , . . and . , . . began to
physically assault each other with closed fist. JCO Tierra Martino, JCO LaToyia Johnson, and JCO Joshua Heriott-
Franklin gave the juveniles several directives to stop fighting. Juveniles continued to fight. Assistance was called.
All available staff entered unit Holly to assist and juveniles were placed back into their correct pods.

I, AUS Officer Jade Scarborough was assigned to unit Holly control on the above date. At the approximate time, I
observed JCO Tierra Martino and JCO LaToyia Johnson secure All pod doors after serving dinner trays. JCO
T.Martino and L.Johnson continued so that they could serve dinner to pod C. I, AUS Scarborough double checked
all pod entrance doors on the control panel and all doors were secured. Captain Eric Favor and Lieutenant
Clarence Adamson entered the unit at 17:34 pm to assist both JCO's with the serving of dinner to all pods When
Captain Favor, Lieutenant Adamson, JCO Martino, and JCO Johnson approached pod C door to serve dinner, I
unlocked pod C door when the button was pushed for entry. As C Pod door was unlocked juvenile

assist.

allowed for this fight to take place. I stood in disbelief for t he conversation I heard. Shortly after t he conversation I
B.I. Jirves advised Captain Favor about what the juveniles were talking about on the yard. Captain Favor
responded "You know I was assaulted ". I advised Captain Favor that I was sorry to hear that. B.I. Jirves advised
that t he youth need to be watched and that the fighting isn't over according to the youth.

On the above date and approximate time I, JSII Latoyia Johnson, was accompanied by JSII Tierra Martino, Cpt Eric
Favor, and Lt. Clarence Adamson to pass out dinner for "C Pod". Cpt Favor opened "C Pod" with his keys and told

he walked away and went to the door of "A Pod" where I saw other juveniles sanding on the opposite side
of the door. While continuin to ass otra -t s I heard a loud noise and at that time I turned my head to the right
and saw juveniles and with closed fists hitting each other in the face.
I then gave severa directives for both juveni es to stop fighting, but at that time all pods were open and no longer

Description
On 6/2/2022 at approximately 13:40, BITM Franklin responded to an assistance call at Holly Unit. When she
arrived there were several youth from A a ~ courtyard. It appeared as if they had just finished
fighting. BITM Franklin approached y o u t h - - and attempted to direct him back to his pod. BITM
Franklin went around and encouraged all youth to return to their assigned areas. Once the youth agreed, Bl
Bankhead held C Pod door opened and C Pod youth proceeded to enter to their assigned pod but all of the youth
in A pod followed behind them . Once in C pod, all of the youth began running towards room 10. Staff followed in
an attempt to deescalate the situation. BITM Franklin and Bl Fischer began trying to stop youth
from going towards room 10. Eventually all of A pod youth exited C POD. At this time, PSO Rochester attempted to
keep C pod youth from leaving out the unit. Insiders Cooper also blocked the door to stop C pod youth from
exiting. BITM Franklin did try to guide some of them back by pulling on the back of their shirts. We were
successful in containing the youth on C pod. They allowed staff to exit the pod without incident. BITM Franklin left
Holly Unit.

On the above date and time I JSII Martino and JSII L. Johnson was preparing for meal pass. I JS Martino secured
all doors and contacted control room to ensure that all doors was secured. At that time, I JSII Martino was called
to control room to speak with Captain Eric Favor about the recreation of Holly C-Pod. I advised Capt. Favor of his
assistance being needed because tensions were high because of a previous event that took place earlier in the
day. Captain Favor and Lt. Clarence Adamson arrived to assist with meal pass. JS Johnson and myself had
already completed A and B Pods but J u v e n i l e - - - was irate and said "Someone is going to get
assaulted" referring to a staff. So, Myself a n d ~ Tli'reatened and decided to allow Captain Favor and
Lt. Adamson to deliver the dinner trays to C Pod. At that time, Captain Eric Favor then used his master set of keys
to open C-Pod. Captain Favor stated that he needed to speak with Juvenile . It was an undisclosed
Conversation. At that time, while delivering dinner trays to C-Pod, A-Pods door was kicked open b a ·uvenile I
cannot identif . The Juveniles that were standin at the door at the time of kicking were juvenile
an . At that time, Juvenile
wast e on y two Juveniles on t e Courtyar . I o served Lt. Adamson trying to secure C-Po s door

physically assaulting each other. Capt Favor, Lt. A a m s o n ~ n d Myself gave juvenile several directives
to stop fighting. I JS. Martino was given a directive from Capt. favor to allow~ h t until assistance
arrived. Juvenile fell to the round and that is when J u v e n i l e - - - - a n d -

Description
On the above date and time I Insiders Coordinator Cooper responded to an assistance call due to youth fighting. I
IC Cooper assisted by providing verbal directives redirecting youth not involved in the fight to return to assigned
pods. As youth were redirected to appropriate locations I IC Cooper observed several youth throw closed hand
fists at each other in room 1 of Holly C Pod. I then continued to provided redirection to unassigned youth to return
to the appropriate pods. When only assigned youth were present in Holly C Pod I provided assistance in keeping
youth on the pod by attempting to pull the door shut and blocked youth attempting to exit.

Description

On the above day and approximate time, Sgt Bates was dispatched to the Holly unit due to youths fighting. Upon
my arrival in 1736, I entered the building at 1737hrs and had a visual of the courtyard. Multiple security staff were
standing on the side of the c o ~ h e exit door while t h ~ g ~ the other side.
Initially i observed that Y o u t h s - - ~ and Y o u t h - - #IIIIIIWere engaged in a
fight as i entered the courtyard. Other Youth in the court yard then began fighting as i accessed the situation. I
then engaged all your in court yard with directives to stop fighting 3 or more times before deploying chemical
munitions in the direction of the youths. Approximately 7-10 youth actively engaged in a physical altercation
ignoring staff directives to stop. Youth were seen with Broken Mop handles, socks with objects inside, and sticks.
After multiple short bursts of chemical munitions the majority of the youth disengaged and dispersed. Security
staff were able to escort effected youth away from the scene. i then ensured A, B and C pods were secured and
Holly Unit was Deemed under control at 1747hrs. Medical was contacted via phone and Security staff was
told that no one could come to medical at that time. Youth was in the infirmary and his injuries
were a priority as EMS would be in route.

Decontamination of youth w ~ n ~ o s e d to chemical munition spray began at 1812hrs at the


nearest water station. Y o u t h - - • - • and y o u t h - - . All youth involved in incident were
identified by BRRC Captain Favors. SLED agents assisted in s e ~ u t h who were placed in mechanical
restraints and transported to laurel unit. Remaining Youth were Advised and allowed to shower.
