1. PURPOSE

The purpose of this Method Statement is to outline the general requirements and
precautionary description of the Foundation Works.

2. SCOPE

Scope of this method statement is to define the sequence and methodology for the
construction involving the following activities:

1. Excavation

2. Dewatering

3. Shoring method

4. Foundation-Rebar-Formwork-Concrete

3. RESPONSIBILITY

3.1 Project Manager and Project Engineer.

Responsible in ensuring the implementation of construction procedure and


method statement, quality control of work, supervise the work and reporting the
progress of work to the OWNER. Then, shall ensure that the required Quality
Control procedures and forms are available in place on a daily basis. Similarly,
they shall ensure that the needed drawings are of the latest revisions.

3.2 Site Supervisor

Supervise closely, the activities designated to them and ensure that all
instructions and safety procedures are followed and strictly adhered to.
Supervise the work to ensure that technical, quality, safety, and purchase
order requirements are met; Attend daily site meetings and communicating
his daily report with the Engineer.

3.2 Site Safety Supervisor.

1. Monitor and supervise the risk assessment.

2. Ensure that necessary Permit to Work is available.

3. Ensure that the project’s method statement and risk assessment are
understood and complied and applied in the worksite.

4. Ensure that all workers are provided with sufficient information regarding
workplace

5. Hazards and planned control measures in ensuring the implementation of


construction procedure and method statement, quality control

4 MANPOWER

The following personnel will be direct and indirect involved in piping fabrication and
installation activities. The number of workers shall be based on the works, volumes and as
where required.

5 EQUIPMENT AND TOOLS

Tools and equipment shall be verified and certified/stickered before deployed on site by
RGTP Inspector. Necessary approval shall be obtained from local authorities as required.
Daily visual inspection shall be conducted by operator/tool user. The following tools and
equipment will be utilized for piping fabrication and installation activities as and when
necessary, to assist the work. The quantity is depending on the scope of works.

6 WORK SEQUENCE FLOW

Preparation works prior to any construction are required to ensure the subsequent
construction action can be performed without much interruption. Those preparation
work shall be listed as below:

Dewatering
Preparation Survey and Excavation
Demolition (if rains)
of works setting out

Subbase
Shoring
Concreating Rebar Work Preparation

7 SITE ACTIVITIES

7.1 MOBILIZATION AND DEMOBILZATION

1. Inspection of machineries and equipment and tools for foundation activities with
IRSO and RGTP. Only fit and inspected equipment are to be utilized for the
work to have smooth and safe work execution. As such those machineries and
equipment are to be inspected for their fitness by OWNER.

2. Mobilization of equipment, material, and tools to work site by using lorry/ pick
up. All the inspected machineries and equipment are to be mobilized to ensure
smooth execution of the works.

3. All the equipment and machineries shall be properly barricaded at work site
using appropriate barricaded markers. Hoarding in some situation is deem
necessary as the hoarding barricade to prevent other workers who are not
associated with the work to enter the work site.

4. Put the signage around the barricade area. The required signages shall be
provided at the work area to alert all the workers. The signages that have been
identified for this excavation work are as follow.

7.2 SURVEY AND SETTING OUT

1. Before work start, contractor should establish baseline and benchmarks from which
co-ordinates and elevations will be taken out the foundation works.

2. Contractor should refer foundation detail RGTP-EHSB-CVS-DTL-001 as the


baseline prior to start any works. Refer attachment 1 for the foundation detail
drawing.

7.3 DEMOLITION

1. Refer to Method Statement : RGTP-P20220019-ESB-CON-JMS-9243-0002_A

7.4 EXCAVATION AND SHORING

1. Prior to commencement of any excavation activities, the approvals for the


following documentation shall be acquired. The required documentations are:

 Excavation certificate
 Permit to Work (PTW)

2. The dimension and method of excavation, whether excavation by hand or


machine shall be clearly indicated on the permit to work (PTW). A foundation
drawing shall be attached to the PTW to indicate which area are to be
excavated. Refer attachment 1 for the foundation detail drawing.

3. Identify nearby existing facilities (foundation, cable and etc) prior to start
excavation work.

4. The backhoe operator and workers shall be briefed on the risk assessment,
method statement and PTW.

5. Excavation will generally be executed in open cut to the size, slope and
elevation shown in the dimension below:

 Length - 13.10 meter


 Width - 7.30 meter
 Depth - 1.25 meter

6. It is estimated that the total of 145 m3 of soil to be removed for the excavation
work. This volume amounted to approximately 215 tons.

7. Excavation will be carried out with excavator and the excavated material
loaded directly into lorry tipper and transport out to the dumped area inside the
RGTP area.

8. A small bucket or manual digging will be used when reaching the final bottom
of the foundation.

9. If an unexpected service is found or if a service is damaged, stop the works/ do


not restart works and liaise with OWNER team immediately.

10. After excavation works has been completed, proceed with next activities while
checking the integrity of shoring from time to time.

11. Used black canvas size 2m x 100m and cut as per dimension of the excavate
area to prevent from the ground collapsing.

12. The excavated ground shall have risk of ground collapsing if without proper
protection or mitigation. For our excavation work, the protection from the
collapsed earth shall be avoided.

13. The subbase shall be compacted to the required compaction requirement. The
manual compactor with 500kg spec shall be utilized for this activity. Crusher
run shall be laid and finally level up with the 50mm lean concrete to set the
bottom level of the foundation.

14. Proceed with construction when the excavation area has been stabilized

7.4 REBAR, FORMWORK & CONCRETING WORKS

7.4.1 REBAR CUTTING AND BENDING

1. Standard length of rebar shall be cut to the required length according to the
approved bar bending schedule/ cutting sheet.
2. Used rebar as mentioned in approved doc RGTP-EHSB-CVS-MTO-001.
3. The cutting machine operator will setup length and quantity of rebar to be cut.
After that, rebar will be grouped per size and diameter.
4. Cut the rebar by using rebar cutting machine as the CONTRACTOR will
fabricate the rebar at their own workshop.
5. The rebar shall be bend as per drawing RGTP-EHSB-CVS-DTL-001.
6. Rebar bending work will be carried out using rebar bending machines.
7. Used galvanized iron wire and welding to create the shape of the foundation as
per drawing and create at their own workshop.
8. Completed cut and bent rebar shall be inspected and approved cut and bend
rebar shall be properly bundled before sent to the work area at RGTP.

7.4.2 REBAR INSTALLATION


1. Lay the bottom bar on 50 mm thick concrete block spacer and tie it to rebar.
2. Steel mold 4m x 2m Qty:1
3. Erect side bar,tie bar,chair bar on the bottom bar and tie them using binding
wire.
4. Erect side bar and tie them.
5. Erect top bar and tie them
6. Once again check the rebar for dimension according to approved drawing.
7. Set side concrete block spacer.
8. Conduct joint inspection to obtain approval from the employer.

7.4.3 FORMWORK INSTALLATION USING CRANE

1. Competent and certified work leader is required to manage the PTW


application.
2. Work area shall be barricade and warning signage have been installed at the
working area to avoid any unauthorized access to the work area.

3. Access the weight of the load to be lifting.


4. Use available lifting aid i.e chain block and lever block where possible. Make
sure the lifting aid is inspected and have valid sticker/inspection tag.
5. Perform visual inspection on the lifting aid. Inspect for wear, twists, broken or
damaged links on chain block.
6. If usage of lifting aid is not possible, request for assistance from other person to
carry the weight together.
7. The simultaneous use of more than one crane or other lifting appliance to raise,
suspend, support or lower a single load should be avoided. Tendon lift required
lifting plan.
8. Lifting work shall begin only after confirming by preliminary check that the
cranes set up is satisfactory.
9. The crane is secured horizontally, and steel plates or square wooden timber
shall be placed under the outriggers firmly, and then secured with knock pins
after set in position. Mobile crane shall be equipped with fix padding for out-
rigging stabilization.
10. Ensure the rebar has been properly attached with the shackle at 4 dimensions.
11. Set the formwork as per layout.
12. Check the alignment of the formworks and ensure to be aligned properly.
13. The crane’s wire rope shall be rolled up entirely after the work is finished and
wire rope shall be route prior to crane movement or relocation.
14. It is necessary to know the center of gravity of a load to be able to balance the
load correctly and distribute the load between cranes as desired. It is also
necessary to account for any conditions that could cause the center of gravity
to shift, such as wind blowing on the load or fluids in or on the load. If the
center of gravity shifts or is not correctly aligned, the crane closer to the center
of gravity could become overloaded, resulting in a failure.
15. Once formworks installation is completed then install the anchor bolts or any
embedded items.
16. The casting level shall be marked at side of the formwork.

17. Formwork shall be dismantled after the concrete hardened.


18. Removed all nails that are exposed on the formworks prior to stacking or
transporting to designated area.
19. Clear all the dismantled items to maintain good housekeeping.

7.4.4 REINFORCEMENT WORK

1. Ensure that rebar is rigidly fixed with galvanized tying wires.


2. Ensure that the reinforcement is in accordance with the drawing specifications.
3. The position of reinforcement shall be checked before and during concreating to
ensure the nominal cover is maintained.

7.4.5 CONCRETING WORK

1. Ensure that all reinforcement and other cast in items are properly installed and
checked.
2. Concrete mixer lorry will pour the ready-mix concrete to the concrete bucket
that had attach to the crane.
3. The dimension of the concrete bucket is 1.2m height x 0.8m width. Pour half of
the concrete to the concrete bucket to prevent overload of the weight.
4. Concrete bucket can support at least 120 kg weight.
5. Using the same method as lifting the rebar for the concrete bucket.
6. Concrete finishing level shall be marked on formwork and verified by the site
engineer. Provide level guide when necessary.
7. During concreting, concrete must be properly vibrated to ensure good
compaction.
8. Level the concrete and finish the surface either with wood or steel trowel
depending on the requirement.
9. After the concrete set, apply curing compound or water for curing.
10. After removing the mold, the extra spacing will pour with crusher run.

8.1 SAFETY AT SITE

8.1.1 All personnel shall attend PGB-RGTP induction to enter PIC. All
employees and subcontractors will be made aware during daily site
toolbox talk of any risks their work can expose themselves and
others in the vicinity. Work will only commence when adequate
protection is in place. Site supervisor will liaise with the OWNER
Construction Team on matter of safety to ensure worker protection.
Assign work leader shall attend RGTP WL Training to become work
leader. All personnel shall obtain valid Working at Height Training
before working at height.

8.1.2 Obtain valid PTW and any other certificate related of the works.

8.2 PROTECTIVE AND SAFETY EQUIPMENT

All workers engaged in the job site shall wear mandatory PPE and provide emergency
medical facilities or kit in accordance to HSE plan.

1. Safety Helmet, Safety Shoes, Safety Harness, and Safety Glasses.

2. Gloves (rubber/semi-leather) suit to activity.

3. High visibility vest/coverall/jacket with reflective band on shoulder, arm,


and leg.

4. Rigger and signalman must use an orange high visibility vest.

5. Ear Plug at classified noise area.

9 . HEALTH & SAFETY PRECAUTION

All personnel are responsible to maintain high standard of cleanliness at workplace.


Workplaces shall be kept in clean, tidy and orderly conditions at all time.
