1. PURPOSE
To ensure that all lifting activities are properly controlled and carried out as per OWNER
and regulatory requirements and is applicable at the point of work. Lifting activities have
been the cause of a relatively high number of industrial accidents. This section outlines
the principal hazards and precautions to be taken in lifting of material within the areas.
2. SCOPE
This procedure provides the general requirements for the lifting works to ensure that the
activities are executed in a safe and systematic manner and in accordance with codes
and standard for Safety and Health for this project.

3. RESPONSIBILITIES
Lifting activities require proper training of personnel involved. Lifting Supervisors must
keep alert for cases off mal-operation including those due to illness. Individuals
performing lifting operations must adhere to established safe procedures and must use
only certified equipment in good condition. Use of other equipment is completely
forbidden.
Crane operators must be competent (DOSH registered) to operate the particular type of
crane. He must also able to:
Responsible for the overall of lifting activities is includes of preparing of lifting plan,
supervision and execution works. To apply crane and lifting request from OWNER
for lifting approval prior lifting work activities can be carried out in the OWNER plant.
Responsible for attaching the load to the crane and must be a competent person
and trained by NIOSH or other recognized institutions. They must coordinate closely
and give lifting instruction to the Signalman prior and during lifting works activities.
Slinger/Rigger can give hand signals to the Signalman but the crane operator will
only follow instruction from Signalman.

Signalman shall be assigned for each lifting project under an operating system
established for the work. They shall stand where they can observe the loading
operation and be clearly visible to the crane operator. A uniform signal system shall
be used consists of flags, transceiver and whistles for signaling. For safety reason,
Signalman shall wear green vest jacket.
Signalman must give the correct hand signals to the crane operator. Standard signal
must be used (refer to Attachments 1). All signals must be given by the Signalman
and crane operator must obey an emergency stop signal given by Signalman or
other responsible officer.
4.1. General
This procedure provides the general requirements for the lifting works to ensure that
the activities are executed in a safe and systematic manner.
For lifting activities, prior to lifting, inspection of cranes and lifting gear are
mandatory. Inspection of crane shall be carried out by authorized OWNER
representatives prior to enter the plant.
The lifting /erection work plan and procedure shall be checked thoroughly by
OWNER.
Warning signs “NO ENTRY”, “ROAD CLOSURE” and “LIFTING ACTIVITY IN
PROGRESS” and safety barricades, safety ropes shall be implemented.
Only authorized persons with license and DOSH registered shall be permitted to
operate the crane or the crane or the slinging for lifting equipment. Lifting
Supervisor shall be experienced and certified.

The foreman and signal man shall be assigned for each lifting project under an
operating system established for the work. They shall stand where they can
observe the loading operation and be clearly visible to the crane operator. A
uniform signal system shall be used consists of flags, transceiver, and whistles
for signaling.
The capacity of the crane to be used for lifting shall be carefully determined with
load identification, verification, and calculation. Load analysis shall be conducted
to check whether the crane and lifting gear are safe prior to lifting.
Total lifting weights and the center of gravity of equipment to be erected and
lifted shall be carefully rechecked before commencing work. This is to determine
location of lifting points.
Before entering the plant and starting the work, the following crane parts are to
be checked: auto load indicator, the conditions and functioning of the brakes limit
switches, over-roll preventing devices, wire ropes, and lifting devices shall be
checked and inspected, and the crane shall be operated on a trial basis without
a load as primary check. In this unloaded state all elements shall be checked
and inspected to see that if they are functioning properly. If risky conditions are
detected, the crane shall not be permitted to perform the activity.
Lifting work shall not be carried out after 1900 hrs and during bad weather, such
as strong winds or rain, storm and thunder/lighting.
All cranes and winches shall be tested and inspected as instructed by OWNER
representative. Cranes or winches shall be locked or braked to secure whilst not
in operation.
Crane shall not move from its location while the boom in the raised position.
Shall not leave the crane with the bottom in the raised position and shall not
leave the crane unattended while engine is running.

Prepare and apply PTW for equipment setup activity with attachment of relevant
documents such as JMS, JHA, etc and get approval from AA.
Competent and certified work leader is required to manage the PTW application.
Work area shall be barricade and warning signage have been installed at the
working area to avoid any unauthorized access to the work area.
Access the weight of the load to be lifting.
Use available lifting aid i.e chain block and lever block where possible. Make
sure the lifting aid is inspected and have valid sticker/inspection tag.
Perform visual inspection on the lifting aid. Inspect for wear, twists, broken or
damaged links on chain block.
If usage of lifting aid is not possible, request for assistance from other person to
carry the weight together.
The simultaneous use of more than one crane or other lifting appliance to raise,
suspend, support or lower a single load should be avoided. Tendon lift required
lifting plan.
Lifting work shall begin only after confirming by preliminary check that the cranes
set up is satisfactory.
The crane is secured horizontally, and steel plates or square wooden timber
shall be placed under the outriggers firmly, and then secured with knock pins
after set in position. Mobile crane shall be equipped with fix padding for out-
rigging stabilization.
Details outline of the rigging plan to be prepared for lifting works e.g. lifting of
equipment, materials, etc., when one of the following conditions prevails.
The load exceeds 75% of the rated load chart capacity for the crane.
The load should not exceed the load chart capacity and exceeds 50% of the
rated load chart capacity for the crane and possible failure would endanger
existing facilities.
Poles or derricks have been erected for a specific lift.
CONTRACTOR shall provide a lifting plan for OWNER approval. Lifting
activities should not be executed without OWNER approval.

The operator shall not leave the crane or winch during lifting work.
The inclined angle of the boom during operation shall not exceeds the range of
30 to 80 degrees unless otherwise specified for the machinery. When using the
jib its length must be minimized. An angle indicator shall be provided for the
crane operator to visually check the bottom angle. Crane bottoms shall be
lowered to the ground level, and the hook shall be secured to the specified
position when the crane is not in use.
The bottoms angle operating angle shall be determine from the available data for
the load and the crane load chart.
During the lifting work, the operation shall be carefully supervised to prevent
hasty lifting, prolonged suspension, and lifting beyond the limit. Abrupt lifting and
stopping shall be prohibited.
Simultaneous rotation and lifting or rotation and boom movement shall be
prohibited.
The boom shall be slowly rotated so as not to produce centrifugal force on
equipment or materials being lifted.
Workers shall be forbidden to ride on lifted equipment or material where lifting or
swinging is taking place.
The crane’s wire rope shall be rolled up entirely after the work is finished and
wire rope shall be route prior to crane movement or relocation.
It is necessary to know the center of gravity of a load to be able to balance the
load correctly and distribute the load between cranes as desired. It is also
necessary to account for any conditions that could cause the center of gravity to
shift, such as wind blowing on the load or fluids in or on the load. If the center of
gravity shifts or is not correctly aligned, the crane closer to the center of gravity
could become overloaded, resulting in a failure.
Repeat with the same step for crane 25 ton.
Half of the weight of the tank will be support by crane 25 ton. Sling belt and
shackle will be used to lift the bottom of the tank.
Lifting supervisor must ensure that the slings are stable and that the load is able
to be lifted the tank.
Supervisors need to ensure that the landing area is adequately prepared.

Never operate the crane when he does not have a clear view of the load, unless
a signalman is suitably positioned to be given required signals (by hand signals or
radio). Only standardized signals should be used.
Never allow any person to ride on a load.
Never leave a load suspended for longer than is necessary and never leave the
crane controls during this time.
Never drag a grounded load.
Always ensure that the hook, load and jib are kept clear of obstacles.
Always avoid suspending or moving the load above personnel and/or beyond the
Work Site fence.
Always lower carefully and smoothly and exercise special care while slowing to
prevent the load from jerking or swinging too fast.
Always obey the emergency stop signal given by Signalman (both arms raised
vertically).
Always lower the jib or mast of a mobile crane sufficiently to pass safety below
overhead cables and pipes or any other obstruction before the crane is moved
from one position to another,
Always secure the hook of a crane in transit to prevent swinging. Install tagline at
suspended load during lifting.
Never exceed the speed limits for a mobile crane on the Terminal (20km/h when
not carrying a load, 10km/h when carrying a load)
Always ensure that the jacks (where fitted on mobile cranes) are screwed down
whenever the load to be lifted exceeds the maximum load permissible without the
use of jacks (as specified by the crane manufactures).
Movement of any crane liable to endanger on overhead cable, pipe or any plant
or equipment must be supervised by a responsible person nominated by the
Operation Supervisor in charge of the execution of the Work Plan.

Utilizing the Crane’s Road Registration Number (Licensee Number) all cranes at
the work site shall be obliged to provide Serviceability Records of the vehicle.
Failing the availability of suitable records, the Crane shall be subjected to an
Inspection by an OWNER authorized person capable and qualified to perform
such Inspections. An all cases the Inspection of the crane shall be witnessed by
the Safety Manager.
The minimum requirements for the certification of a Crane shall be as follows:
A valid License and Road Tax Certificate.
Vehicle to be road worthy and a clean, tidy condition.
Have available all relevant load rating charts shall be legible and up to date.
All lifting tackle, wire rope or slings, hooks, shackles, etc. shall be in good
condition.
Crane rate load capacity charts, recommended operating speeds, hazards,
warning shall be conspicuously posted inside the Operator cab.
All vehicle lights shall be in working order including the roof top “READ’ rotating
hazard light.
A Fire Extinguisher with a minimum rating of at least 10BC, shall be available in
the cab of each crane.
When equipped with outriggers, they shall be checked for serviceability.
All winch brakes, clutches, Safety latches, and etc. shall be in a serviceable
condition.
When operating under full load the vehicle shall not emit exhaust smoke over the
specified limitation.
Spark arrestor fitted while entering the plant area.
Color coding system to be implemented.

All cranes shall be fitted with an appropriate safe working load indicator and loading chart
shall be placed inside the crane where the operator can easily see it.
The indicator shall be of such a type that the operator can readily ascertain the safe
working load of the crane at the boom length and radius at which he is working.
6. INSPECTION REQUIREMENTS
CONTRACTOR has to submit the required documents to OWNER before carry out crane
and inspection at least 24 hours prior inspection day. Crane’s company has to submit the
following documents for the inspection to CONTRACTOR for submission to OWNER as
follows:
Crane driver’s license
Competent Crane driver’s with DOSH registered
Crane Certificate (PMA Certificate) from DOSH
Crane Road Tax and Insurance
Crane specifications i.e. capacity, crane lifting chart, model, made etc

Inspection lifting gear is require to ensure their integrity and on going safe operation for lifting
operation. Lifting gear inspections are performed on a regular basis depending on the
standards and specifications of the equipment and its usage. Generally 6 monthly inspection
is used as the guideline. These inspection should be conducted by competent inspector and
they should approve the items prior to be use.
All related lifting gear documents has to submit to owner prior to inspection. Types of lifting
gear to be use during tank lifting are as follows:

8. HOUSEKEEPING
The housekeeping needs to emphasize to maintain the worksite in safe place for all
workers. Before, during and after all the site activities workers need to ensure
housekeeping is carried daily and worksite to be neat and tidy.

Individuals responsible for directing crane and hoist operators during lifting operations
shall use standard hand signals (Attachment 1A). These signals shall be conspicuously
displayed in work areas to ensure proper use.
Stop With forearm extended to the side and palm down, hold
position rigidly.
Emergency stop With arm extended to the side and palm down, move hand
rapidly right and left.
Bridge travel With forearm extended forward, and hand open and
slightly raised, make pushing motion in the direction of
travel.
Trolley travel With palm up, fingers closed, and thumb pointing in the
direction of motion, jerk hand horizontally.
Move slowly Use one hand to give any motion signal, and place the
other hand motionless in front of the hand giving the
motion signal.
Multiple trolley cranes Cranes that are equipped with two separately operated
trolleys present the problem of establishing signal person
to operator signals when both trolleys are being operated
to accomplish the work at hand. Thus, crane trolleys
should be numbered with numerals large enough to be
legible from the floor. For example:

■ 定格総荷重表注意事項
1. 定格総荷重表は、水平堅土上において機体を水平に設置した状態での保証できる最大荷重を示
しており、フックその他のつり具等の質量を含んだ値です。
部分は機械の強度によって定められ、他は機体の安定度によって定められています。
2. 作業半径はブームのたわみを含んだ実際の値にもとづいていますので必ず作業半径を基準とし
て作業を行ってください。
3. ジブの作業半径は 43. 0m ブームにジブを装着して作業を行った値を示し、その他のブーム長さで
のジブ作業はブーム角度だけを基準として行ってください。
4. アウトリガ最縮小張出では、ジブ作業を行わないでください。
5. アウトリガ張出状態によって側方領域でのつり上性能は異なります。従って各々の張出状態に
おける定格総荷重表で作業を行ってください。前方、後方領域でのつり上性能はアウトリガ最
大張出の定格総荷重表で作業を行ってください。

3. ルースタシーブの定格総荷重は、ブームの定格総荷重より 22 トンフックの質量(320kg)を
差引いた値と等しく、かつ限度を 4, 500kg とします。

4. ブーム長さ 23. 2m をこえるブーム作業、ジブ作業は行わないでください。
5. 定置つり作業はパーキングブレーキを効かせ、同時にブレーキロックを掛けた状態で行ってく
ださい。
6. 走行つりはシフトレバーを 1 速にして超低速スイッチを押して行ってください。
7. 走行つりは荷が振れないように地面近くに保持し、2km/ h 未満にて行ってください。特にコー
ナリング、急発進、急制動に注意してください。
8. 走行つり中にはクレーン作業は行わないでください。また、旋回ブレーキを必ず掛けてくだ
さい。
9. 上記項目以外はアウトリガ使用時注意事項の 2、7、9、10、12、13 の各項目に従って作業を行 ってくだ
さい。
