filming will be in a face-to-face setting on September 11, 2022, at 2 PM at Caraga Regional
Science High School.

This is to further certify that I/we have understood the benefits the child could derive
from participating in this activity and that my/our permission is voluntary and discretionary.
Further, it is known to my/our part that the administrators are not held liable to any untoward
incidents despite the precautionary measures and safety we have observed.
