AUDIT OBSERVATIONS:

3rd FLOOR GENERAL WARD

 Have to check on general ward bed distance

3rd FLOOR

 Private room 201 AC and fan was not switched off, miscommunication
between nurses regarding discharge to security

2ND FLOOR

 Wheel chair in second floor - it was not cleaned and also it needs to be
repaired
 Trash cleaning of private room 102 was not done
 There should be defibrillator tag in OT
 Sterile unsterile instruments keeping system have to be sorted

1ST FLOOR

 X-ray room BMW dustbin is not there dental x-ray testing items are left very
untidy and infectious
 Wiring system of x-ray room has to be fixed
 Lab Lj chart is maintained, it should be verified by the in-charge and sealed
 In lab we have to keep out sourced samples TAT and in house TAT also it will
be great if we can give them readymade registers for our sourced samples

GROUND FLOOR:

 Phlebotomy register is never closed

 Consultation room number 3- switch board was open and fan was not
working- now fixed.
 Pharmacy Formulary yet to be prepared- in process
 Early Warning Sign Chart have to ordered and used for all in-patients.
