AFFIDAVIT OF EXPLANATION
I, VIVIEN MARGARETTE DE LEON, of legal age, Filipino and with
residence and postal address at E-12 RSG Guevent Homes Policarpio St. San
Andres Cainta Rizal, after having been duly sworn to in accordance with law
hereby depose and say:

1. That I am the Legal Assistant of Atty. Michelle De Leon Mediante who

is the attorney at law in a Notary Public located at 107 A A. Bonifacio
Ave., Brgy. Sto Domingo Cainta Rizal with Roll of Attorney No. 61278,
IBP No. 171386/01-03-2022, PTR No. 8234786/01-03-2022;

2. That on June 24, 2022 when I was recording/printing documents, I

erroneously put the wrong document number instead of 479 it’s been
recorded as document 279:

3. That due to the said errors, documents have been recorded wrong
document number.;

4. That efforts are still exerted to have the same located but if presented,
we will be able to identify if the same was duly notarized by our office;

5. That I am executing this affidavit in order to attest to the truth and

veracity of the foregoing statements, and for Attest and to explain for
all intents and legal purpose it may serve.
