5.1 Head Engineer (HE)
5.1.1 Head Engineer should set goals, plan, organize, and control the activities under his
jurisdiction
5.1.2 All goals should be specific, well-defined, quantifiable, with an estimated time of
achievement given for each goal
5.1.3 Each goal should be communicated freely and clearly to all those involved
5.1.4 Goals should be reviewed regularly with Shift Engineer and maintenance representatives
5.2 Shift Engineer (SE)
5.2.1 Shift engineer has a responsibility to support the Head Engineer in achieving the set goals
5.2.2 Goals set are achieved through conducting maintenance operations and ensuring that the
hospital’s engineering policies are upheld by supervising a team, monitoring preventative
maintenance procedures, taking purchase orders, keeping records, and controlling
inventory.
5.3 Maintenance Staff (MS)
5.4 Organization
5.4.1 Electrical, Mechanical, Plumbing, and Fabrication section must share responsibility in a
coordinated effort to optimize facility performance
5.4.2 Electrical section has a responsibility to provide a safe, working, and consistent electrical
system while maintaining an energy-efficient facility
5.4.3 Mechanical section has a responsibility to maintain the ventilation, refrigeration and cooling
system of the hospital
5.4.4 Plumbing section has a responsibility to ensure that all water supply, sewage, and storm
drain system are working properly without operation disruption and maintain that the
sewage treatment plant functions as recommend by the DENR

7.0 Procedures
7.1 Building Inspection
7.1.1 Head Engineer and/or Shift Engineer conducts weekly inspection of hospital interior and
exterior in coordination with members from General Services group. All findings are
documented and categorized according to location, field of concern, and urgency. Reports
are submitted to Chief Operating Officer and members of the top management
7.1.2 Shift Engineer makes, assigns, and distributes job orders containing the inspection findings
to each maintenance section sorted per priority for possible repairs. Deadline is assigned for
each job order to maintain regulate the turn-around time. Shift engineer raises purchase
request if materials needed for job order is not available and deadline is adjusted as needed.
7.1.3 Shift engineer monitors job assignment during repairs to ensure the quality of work and that
the deadline is met. Once job is completed, shift engineer verifies the completed work and
updates the Head Engineer.
7.1.4 Completed works and updated on-going works are reported by the Shift Engineer to the
Head Engineer. Shift engineer will also report if which works will be marked as pending and
shall give remarks as to the reason of delay.
7.2 Work Orders
7.2.1 All Departments shall have access to the Online Ticketing System (OTS) for use in requesting
repair works in their respective departments. If repair works was requested apart from using
the OTS, the staff responsible from receiving the request should relay to the requester to
input the request on the OTS.
7.2.2 All activities of the MS with regards to repair request shall be pursuant of an online ticket.
7.2.3 The HE or SE shall organize the tickets according to urgency and importance. The HE or SE
shall also set the deadline for the ticket to be completed.
7.2.4 The HE or SE may delay the priority order of a ticket per his discretion. The departments
affected by the delay must be informed by the HE or SE
7.2.5 Tickets are then sorted according to Maintenance and then assigned MS for appropriation of
needed repair works.
7.2.6 After assigning the ticket to the appropriate section, the staff assigned shall proceed to the
requester to complete the work order request. If additional materials needed are not
available, the assigned staff shall raise a purchase requisition for material procurement.
7.2.7 The MS shall complete the work order based on the information indicated in the online
ticket within the set deadline. The MS shall report the work order to the SE after
completion.
7.2.8 If the ticket has passed its set deadline, the MS shall explain to the SE the reason for delay.
Possible reasons for delay may include but not limited: availability of material, conflict of
schedule, emergency works.
7.2.9 The SE will check all tickets declared as completed by MS. If the work completed is deemed
acceptable by both requester and SE,
7.3 Electrical Preventive Maintenance
7.3.1 Refers to maintenance procedure performed on regular basis to prevent damage and
malfunction to electrical data connections
7.3.2 Facilities shall be inspected daily
7.3.2.1 Examine outlets for signs of burning and check for possible replacement
7.3.2.2 Check for signs of burn and cracks of light switch and replace as needed
7.3.2.3 Check for busted bulbs and signs of darkening of light fixtures and replace as needed
7.3.2.4 Check for exposed wirings, scratching of insulation, and signs of burning of electrical
wirings
7.3.3 Panel boards, sub-meters, circuit breaker, and distribution panels shall be checked monthly.
7.3.3.1 Check thoroughly the interior and exterior
7.3.3.2 Check function of all meters and breakers
7.3.3.3 Collect and record meter reading
7.3.3.4 Verify fuse rating and continuity, and load
7.3.3.5 Conduct visual inspection for burns, deterioration, and cracks
7.3.3.6 Clean dust and debris using brush
7.3.3.7 Apply contact cleaner for lubrication
7.3.3.8 Tighten bolts and nuts of connections
7.3.4 Electrical rooms shall be checked monthly
7.3.4.1 Cleaning of room and removal of hindrances to access

7.5 Plumbing Maintenance
7.5.1 Water Supply Monitoring
7.5.1.1 Plumbers shall collect data of meter reading for Laguna well inflow and Laguna well
outflow. Monitoring reports shall be attested by SE

7.5.1.3 Pump motors and control for water supply distribution shall be inspected weekly
7.5.1.3.1 Check for noisy bearing and other noise
7.5.1.3.2 Check pressure switch and gage for functionality

shaft seal
7.5.1.3.4 Check for loose screw and release stored pressure by opening suction
motor plug

7.5.2.1 Septic tank must be cleaned and inspected quarterly.
7.5.2.2 Clean the tank residue through suction by using submersible pump.
7.5.2.3 Clean the filter and replace if needed.

7.5.2.5 Checking of pipe and union connection for damages and replace as needed
7.5.2.6 Checking of float switch with control for damages and loose connections
7.5.3 Gutter, Downspout, and Storm Drain
7.5.3.1 Clean regularly all gutter to prevent clogging
7.5.3.2 Check for leaks and apply sealant as needed

7.8.1 Clean the equipment.

7.8.6 Inspect seals and fix leaks.
7.8.7 Check interior lights.
7.8.8 Clean coils.

7.8.10 Check refrigerant levels.
7.8.11 Observe motor operation.
7.8.12 Examine all parts for wear and tear before a minor problem becomes a big problem.
7.8.13 Check suction line insulation.

7.8.15 Inspect compressor operation. Compressor failure can prompt the replacement of the
refrigeration unit, which makes compressor operation checks a priority on the refrigeration
preventative maintenance checklist.
7.8.16 Check electrical connections
7.8.17 Test thermometers and recalibrate if necessary.
7.9 AHU
7.9.1 Switch OFF the AHU to be maintained.
7.9.2 De-energize the circuit breaker of the unit to be maintained.
7.9.3 Open the cover of the AHU to access the blower and motor assembly.
7.9.4 Verify and check the alignment of Blower and Motor Pulley.
7.9.5 Check the Bag Filters. Replace if found dirty.
7.9.6 Inspect the condition of drain pan. Clean any foreign material and dust that would
contribute to clogging.
7.9.7 Check the condition of the drain pipe. Apply air pressure to remove any slime or foreign
materials inside the pipe.
7.9.8 Inspect the V-belt condition. If damage is found replace it with the same type.
7.9.9 Apply specified amount and quality of grease on the motor and pump.
7.9.10 Switch ON the unit.
7.9.11 Check for any undue noise on the blower and motor. Rectify if found any
