STATEMENT OF CLAIM

1. The Plaintiff is a Malaysian citizen with NRIC Number XXXXXX-XX-XXXX and having his
residential address at 1, Lorong Dua, Kuala Lumpur, Wilayah Persekutuan Kuala Lumpur.

2. The Plaintiff is the sole proprietor of Awal & Akhir Enterprise.

3. The Defendant is a company incorporated under the Companies Act 1965, with its registered
address being Wisma Murni, Kuala Lumpur, Wilayah Persekutuan Kuala Lumpur

4. The Defendant is in the business of processing and manufacturing beverages,

5. On the 21.06.2016, the Plaintiff was appointed by the Defendants as an agent for the sale of goods in
the state of Melaka

6. As per the agreement, the Defendant agreed to pay commission at the rate of 7 percent of the selling
price of the goods to the Plaintiff.

7. Referring to paragraph 6 herein, clause 8 of the agreement states that the Defendant shall furnish an
account of sales achieved by the agent and that the Defendant shall pay the commission by the fifteenth
day of the following month

8. The Plaintiff avers that the Defendant has failed to deliver any account of sale and pay any
commission which is due to the Plaintiff

9. Referring to paragraph 8 herein, The Plaintiff claims that the Defendant has breached the agreement
and now claims for the account of all sales and the amount of commission due, together with interest

10. The Plaintiff pray to the court and claims against the Defendant for
a) The amount of commission due together with interest
b) Interest
c) Costs
d) Such further relief that the this court deems fir and proper for the above situation
