I, Engr. Aizon L. Susulan, is currently residing at 37-J Baliwasan Grande, Zamboanga City has passed the
LICENSURE EXAMINATION FOR MASTER PLUMBERS last February 15-16, 2022. I was not able to attend
the virtual OATH-TAKING CEREMONY last March 18, 2022 that was spearheaded by PRC- Lucena through
Microsoft Teams because of the unavailability of the E-Oath Appointment for Master Plumbers in my
PRC official account.

In line with this, I would like to request in your good office to allow me to have an individual/special oath
at your most convenient time.
