Introductory Message
For the facilitator:

Welcome to the Health 9 Alternative Delivery Mode (ADM) Module on


Community and Environment Health Project Assessment

This module was collaboratively designed, developed and reviewed by educators


both from public and private institutions to assist you, the teacher or facilitator
in helping the learners meet the standards set by the K to 12 Curriculum while
overcoming their personal, social, and economic constraints in schooling.

This learning resource hopes to engage the learners into guided and independent
learning activities at their own pace and time. Furthermore, this also aims to help
learners acquire the needed 21st century skills while taking into consideration
their needs and circumstances.

In addition to the material in the main text, you will also see this box in the body
of the module:

As a facilitator you are expected to orient the learners on how to use this module.

The hand is one of the most symbolized part of the human body. It is often used
to depict skill, action and purpose. Through our hands we may learn, create and
accomplish. Hence, the hand in this learning resource signifies that you as a
learner is capable and empowered to successfully achieve the relevant
competencies and skills at your own pace and time. Your academic success lies
in your own hands!

This module was designed to provide you with fun and meaningful opportunities
for guided and independent learning at your own pace and time. You will be
enabled to process the contents of the learning resource while being an active
learner.

What I Need to Know

A healthy community reflects a sense of well-being. It is


the foundation for achieving all other goals and is essential for
a productive society. Thus, it also helps in building our
country’s economy and in equipping our students to be
healthier in order to learn and succeed academically.
In this module you will encounter social issues and
problems involving the threats of community and environmental
destruction. You will be encouraged to get involved in programs
advocating community and environmental health.
After going through this module, you are expected to:
• Participates in implementing an environmental project
such as building and maintaining a school garden or
conducting a war on waste campaign (depends on
feasibility).

What I Know

PRE-ASSESSMENT
Can you still recall about your school? Close your eyes (in 10
seconds) while opening your memories in the school where you
were enrolled at. Then slowly open your eyes while keeping at
the same time the scenario of your school. Read and analyze
each item that follows.
Visit your school canteen. Find out if the school canteen
complies with the requirements on food sanitation. Here’s the
checklist you need to accomplish by checking the column
under “YES” or “NO” according to health requirements column.

2. Study on the Problems on the Drainage System of the
Barangay or your respective locality
3. Promulgate Laws for Proper Waste Disposal
4. Initiate a Signature Campaign Against Illegal Mining and
Coral Reef Degradation

Your action plan will detail your involvement in protecting your


environment. The safety of the community is the primary
concern of the government. You are encouraged to link to the
nearest NGO’s and GO’s for the support of this program.

This template was being introduced to you last meeting and


you were instructed to complete the parts of the table. The
abovementioned four (4) suggested activities can be
very helpful to you in completing this template. Do not forget
to take pictures during the process of the action
implementation. You can use any available camera you have at
home. Write down your plans of action and strategies. Review
your input and be ready to present your answers to your
teacher!

Assessment

The completed individual action plan output will be collected


by the teacher and be checked. Documentation through
pictures is encouraged for easy verification of the project
assessment. Such documentation is taken during the process
of the action plan. Pictures are to be sent through a group chat
